THE EVALUATION OF CYTOKINE STATUS IN CHILDREN SUFFERING FROM RECURRENT RESPIRATORY VIRAL INFECTIONS

Aim: It is accepted to carry children subject to often respiratory diseases into the group of frequently ailing children (FAC). Often diseases adverb affect on health of children, causing chronic pathology of various organs and system. The aim of our work was to detect cytokine status of FAC. Methods: With this purpose we examined 96 children frequently ailing with Acut Respiratory Viral Infections. Children have been examined in an acute period of disease and during remission. At all children we have defined the levels of IL-1β, IL-2, IL-6, 1L-8, TNF-α and INF-γ in serum with methods of immunochemoluminescence and IFA. Results: It was revealed, that in children frequently ailing with acute respiratory infections, in the acute period of the disease increase of levels of proinflammatory cytokines IL-1β, IL-6, IL-8, TNF-α and decrease of levels of IL-2 and INF-γ was marked. Clinical remission in these children is not accompanied by normalization of cytokine status. The high level of proinflammatory cytokines, testifies to, proceeding of inflammatory process that is possible connected with persistantion of the infections agent. Acute decrease in the levels of IL-2 and IFN-γ in these children, most likely, is caused by the presence of a secondary immunodeficiency of cellular type. Decrease in the number of T-cells also, testifies to it. Decrease in the number of T-cells can be explained with reduction of IL-2- production-the growth factor of T-cells. Conclusion: In this connection it is necessary to carry out an adequate therapy of FAC with acute respiratory viral infections.

Amaç: Viral enfeksiyon ajanları ile sık hastalanan çocukların sitokin statüsünü saptamak ve sitokin durumunda olan değişiklerin solunum sistemi hastalıklarına etkisini araştırmaktır. Yöntem: Sık hastalanan 96 hasta ve sağlıklı kontrol olarak da 30 çocuk çalışmaya alındı. Bütün çocuklarda serum IL-1β, 1L-2, IL-6, IL-8, TNF-a ve İFN-γ-seviyeleri, çalışma grubunda hastalığın akut döneminde ve iyileşme döneminde olmak üzere 2 kez olacak şekilde ölçüldü. Bulgular: Hastalarımızın %52’si erkek, %48’i kızdı. Hastalarımızda akut pnömoni %22,2 hastada, akut obstruktif bronşit %27,2, rekürren bronşit %50 saptandı. Yirmi bir çocuğun postero anterior akciğer qrafilerinde pnömonik infiltrasyon saptandı. Bütün çocuklarda, periferik kan yayması, C-reaktif protein, beyaz küre sayımı ve sedimentasyon bakıldı. Boğaz kültürü ve orofaringeal lavajda virolojik çalışma yapıldı. Sık hastalanan çocuklarda hastalığın akut döneminde ve iyileşme döneminde kontrol grubuna göre proinflamatuar sitokinlerden IL-lβ, IL-6, TNF-a, IL-8-in serum seviyeleri yüksek, hücresel immunitenin başlıca belirteci olan IL-2 ve IFN-γ-nın seviyelerinin ise düşük olduğu saptandı. Sonuç: Viral enfeksiyon ajanları ile sık sık hastalanan çocukların sitokin durumunun kliniğin gidişinde önemli rol oynadığı düşünüldü.
