We aimed to evaluate the effects of intrathecal sufentanîl added to hyperbaric bupivacaine for lovver extremity sur¬gery. After obtaining approval from the ethıcs commîttee, fıfty patients (ASA l-ll) vvere Included in this study and were allocated randomly to two groups of 25 each. 10 mg hyperbaric bupivacaine+10 fjg sufentanil was injected to Group S and the same dose of hyperbaric bupivacaine given to Group K with 2 mL %0.9 NaCi four mL solution was used in both groups for İT anesthesia. Heart rate (HRj, systolic blood pressure (SBP), diastolic blood pressure (DBPj, mean blood pressure (MBP), peripheral oxygen saturation (Sp02İ, sensory and motor block vvere recorded every 5 minutes during fırst'30 min, every W min for the follovving 30 min, and every 15 min intervals in the operation period. Intraoperative/postoperative side effects and postoperative analgesic requirements vvere recorded. HR, SBP, DBP and MBP vvere decreased signifıcantly in both groups vvhen compared vvith control values and no significant difference vvere noted betvveen the groups. The onsettime of sensory block was shorter in group S and sensory block was high-er in Group S than Group K. The duration of anaigesia was longer in Group S than Group K (p<0.05). İn conclusion; intrathecal W fjg sufentanil added to hyperbaric bupivacaine for lovver extremity surgery increased the duration of anaigesia İn the early postoperative period. 10 ug sufentanil plus hyperbaric bupivacaine provided to improve both the guality and duration of anaigesia in this procedures. Because this combination had no adverse effect except pru-ritus, we concluded that it can be used safety in lovver extremity surgery.

Bu çalışmada alt ekstremite cerrahisinde intratekal (İT) hiperbarik bupivakaine eklenen sufentanilin etkilerinin araştırılması amaçlandı. Etik komite onayı alındıktan sonra çalışmaya ASA l-ll sınıfına uyan 50 olgu alındı ve randomize olarak 25şer kişilik iki gruba ayrıldı. Grup S'e 10 mg (2 mL/10 mgj hiperbarik bupivakain+W pg (2 ml/i 0 pg) sufen¬tanil ve Grup K'ya aynı miktardaki hiperbarik bupivakain 2 mL %0.9 NaCi ile verildi. Her iki grupta da 4 mL volüm İT anestezi için uygulandı. Kalp atım hızı (KAH), sistolik arter basınçları (SAB), diasto/İk arter basınçları (DABJ, ortalama arter basınçları (OABj ve periferik oksijen satürasyonları (Sp02). sensoryal ve motor blok ilk 30 dakikada her 5 dakikada bir, takip eden 30 dakikada 10 dakikada bir, sonraki operasyon periyotlarında ise her 15 dakikada bir kaydedildi. İntrao-peratif ve postoperatif yan etkiler ve postoperatif analjezik gereksinimleri belirlendi. Kontrol değerine göre her iki grup¬ta KAH, SAB, DAB, OAB anlamlı derecede düştü, fakat gruplar arasında bu parametreler açısından fark yoktu. Grup S'de sensoryal blok başlama zamanı daha hızlıydı. Analjezi süresi Grup S'de Grup K'ya göre daha uzun bulundu (p<0.05}. Sonuç olarak; alt ekstremite cerrahisi için İT hiperbarik bupivakaine i 0 pg sufentanîl ilave edilmesi bu işlem¬lerde hem analjezi kalitesini artırmış hem de erken postoperatif analjezik gereksinim zamanını uzatmıştır. Bu kombi¬nasyonla kaşıntı dışında anlamlı bir yan etki görülmediğinden alt ekstremite cerrahisinde güvenle kullanılabileceği kanaatine varılmıştır.
