THE EVALUATION OF PERIAPICAL STATUS AND QUALITY OF ENDODONTIC TREATMENT ON A GROUP OF PATIENTS EXPERIENCED ENDODONTIC TREATMENT

Purpose: The purpose of this study was to evaluate periapical status and quality of root canal treatment in a group of patients experienced endodontic treatment. Material and Method: Digital panoramic radiographs of 248 patients (147 females, 101 males) aged 16 or over were examined. Root-filled teeth were classified according to the type of coronal restorations. Periapical status of the teeth was evaluated by the Periapical Index (PAI) Scoring System proposed by Orstavik et al. Quality of root canal treatment was evaluated according to the scoring system proposed by de Moor et al. Obtained data were statistically analysed by using descriptive statistics and chi-square tests. Results: Totally 5969 teeth in 248 patients were examined and 574 teeth (9,6 %) had root canal fil- lings. Of the root filled teeth, 57,5 % showed healthy periapex and 42,5 % was found to have apical perio- dontitis in different grades, where 66,4 % were deter- mined as good quality and 33,6 % were determined as poor quality root canal treatment. Statistically signi-ficant correlation was found between periapical status and quality of root canal treatment (x2=338,588; p<0,001). There was statistically significant correlation between coronal restoration type and periapical status (x2= 16,549; p<0,01). The teeth with intracanal posts (62,4 %) had statistically significant high periapical pathosis than teeth without posts. Conclusion: According to the results of this study, there was a significant correlation between the periapical status of the root-filled teeth and quality of root canal treatment. Also, coronal restorations and the use of intracanal posts can be an important factor in the treatment success.

Amaç: Bu çalışmanın amacı, endodontik tedavi görmüş bir grup hastadaki dişlerin periapikal durumu ve yapılan endodontik tedavilerin kalitesini değerlendirmektir. Gereç ve Yöntem: Çalışmada 16 ve daha üzeri yaşlardaki 248 hastanın (147 kadın, 101 erkek) dijital panoramik radyografları değerlendirildi. Endodontik te¬davi görmüş dişler koronal bölgedeki restorasyonlarına göre sınıflandırıldı. Dişlerin periapikal durumu Orstavik ve arkadaşları tarafından önerilen Periapikal İndeks (PAI) Skorlama Sistemi, kök kanal tedavisinin kalitesi ise de Moor ve arkadaşları tarafından önerilen skorla- ma sistemine göre değerlendirildi. Elde edilen veriler tanımlayıcı istatistikler ve ki-kare testi kullanılarak istatistiksel olarak analiz edildi. Bulgular: 248 hastada toplam 5969 diş incelendi ve bu dişlerin 574'ü (% 9,6) kök kanal tedaviliydi. Kök kanal tedavili dişlerin % 57,5'i sağlıklı iken, % 42,5'inde farklı şiddette apikal periodontitis belirlendi. Değerlendirilen kanal dolgularının % 66,4'ü yeterli kalitede iken, % 33,6'sının kalitesi yetersizdi. Çalışma- da kök kanal tedavili dişlerin periapikal durumu ve kök kanal tedavisinin kalitesi arasında anlamlı bir ilişki (x2= 338,588; p<0,001) bulundu. Kök kanal tedavili dişlerin koronal restorasyon tipi ile periapikal durumu arasında anlamlı bir ilişki vardı (x2= 16,549; p<0,01). Kanal içi post kullanılan dişlerde (% 62,4), kullanılmayanlara göre istatistiksel olarak anlamlı düzeyde fazla periapikal patoloji izlendi. Sonuç: Bu çalışmanın sonuçlarına göre endodontik tedavili dişlerin periapikal durumu ile kanal dolgusunun kalitesi arasında anlamlı bir ilişki vardır. Aynı zamanda yapılan koronal restorasyonlar ve kanal içi post kullanılması endodontik tedavinin başarısını etkileyen önemli etkenlerdendir.
