ÇÖLYAK'LI TÜRK ÇOCUKLARINDA HLA İLE HİPERTRANSAMİNAZEMİ VE ANTİGLİADİN DÜZEYİ İLİŞKİSİ

We aimed to study the relation between liver injury or gliadin antibodies and human leukocyte antigens. The 30 celiac disease patients were included in our study. The mean age of study population was 5.8 years; 20 being female. The HLA-A, -B, -DR and -DQ antigens were studied by the lymphocytotoxic reaction. Results.- The children aged 2 years and below with gliadin antibodies above 50 AU, were not positive for HLA-A32, -B 16, -B5, -DQ4, -DQ6(1) and -DR14(6) antigens. HLA-B4 antigen was more frequent and HLA-DR7 was significantly absent among the children older than 2 years who also had gliadin IgA antibodies above 50 AU. Similarly, among the children above 2 years with IgG gliadin antibodies above 50 AU, HLA-A1, -A9, -B17, -B37, -B73 and -DR9 had highly significant negative association with celiac disease. The celiac patients who had HLA A11; -B18, -B40 and -DQ1 did not show aspartate aminotransferase elevation. Conclusion.- We conclude that there is a relation between human leukocyte antigens and aminotransferases and gliadin antibody levels.

Bu çalışma, toplumumuzdaki çölyaklı çocuklarda karaciğer tutulumu ve antigliadin antikor düzeyi ile doku grupları arasındaki ilişkiyi araştırmak için planlandı. Araştırmaya Cerrahpaşa Tıp Fakültesi Çocuk Gastroenterolojisi Bilim Dalı'nda izlenmekte olan çölyak hastalığı tanılı 30 hasta alındı. Hastaların 20'si kız, 10'u erkek olup yaş ortalaması 5.8 yaş idi. Doku tipi olarak HLA-AB, -DR, -DQ antijenleri lenfositotoksisite yöntemi ile araştırıldı. Tanı konma yaşı iki yaşın altında olup antigliadin Ig => 50 AU olan hastalarda HLA-A32, -B16, -B5, -DQ4, -DQ6(1) ve -DR14(6) doku gruplarına rastlanmadı. İki yaşın üzerinde olup antigliadin Ig > 50 AU olan hastalarda ise HLA-B4'ün daha sık, HLA-DR7'nin ise daha az olması anlamlı bulundu. İki yaşın üzerinde olup antigliadin IgG>50 AU olan hastalarda ise HLA-A1, .-A9, -B17, -B37, -B73 ve -DR9 doku gruplarının olmaması dikkat çekici bulundu. HLA-A11, -B 18, -B40 ve -DQ1 içeren çölyaklı hastalarda anlamlı olarak aspartat aminotransferaz yüksekliğine, HLA-A19, -B18 ve -B44(12) içeren hastalarda ise anlamlı olarak alanin aminotransferaz yüksekliğine rastlanmadı. Sonuç olarak doku grupları ile transaminazlar ve antigliadin düzeyleri arasında bir ilişki bulunmakla beraber, bu sonuçların başka prospektif çalışmalarla da desteklenmesinin yararlı olacağı düşünüldü.
