Peripheral blood lymphocyte subgroups and activation markers in bronchial asthma are studied mainly in cases with allergic or mixed etiology. This study investigates peripheral lymphocyte subgroups and activation markers in allergic and non allergic asthma cases and compares the results with suitable controls.
Results.- The study population consisted of 16 patients with allergic (A) asthma (10 women, 6 men with a mean age of 32.4 ± 9.1 SD years); 14 cases with non allergic (NA) asthma / 10 women and 4 men with a mean age of 33.2 ± 7.8 SD years) and 11 healthy (H) controls (5 women and 6 men with a mean age of 32.9 ± 8.9 SD years). CD3 and CD4 were found to be increased in all (A and NA) asthma patients (p<0.01) whereas CD19 and CD40 were found to be elevated only in A group (p<0.01). In NA group there was an increase in CD45 RO and a decrease in HLA DR.
Conclusion.- Total T lymphocytes and helper T lymphocytes are increased in all asthma patients. Allergic asthma patients have elevated B lymphocyte markers (CD19, CD40); non allergic asthma patients have increased memory T lymphocytes.

Stabil bronş astmasında periferik kanda lenfosit altgrupları ve aktivasyon belirleyicileri ile ilgili yapılan, çeşitli terapötiklerin etkilerini veya diğer hastalık grupları ile ayırımını gösteren pek çok çalışma bulunmaktadır. Bu çalışmalarda sadece allerjik grup veya karışık olgular kullanılmaktadır. Yöntemlerin doğruluğunu irdeleyebilmek amacı ile allerjik ve allerjik olmayan İstanbul'lu astmalı olgularda ve sağlıklı sigara içmeyen erişkinler ile kontrollü olarak, lenfosit altgrupları ve aktivasyon belirleyicileri araştırılmıştır.
Allerjik grup 10 kadın, 6 erkek, yaş ortalaması 32.4±9.1; allerjik olmayan grup 10 kadın, 4 erkek, yaş ortalaması 33.2±7.8 olup; sağlıklı erişkin grup 5 kadın, 6 erkek yaş ortalaması 32.9±8.9'dur. Allerjik olgular sadece ev tozu akarlarına anamnez, deri testi ve sIgE ile kanıtlanmış allerjisi bulunanlardan seçilmiştir. Astmatik olgular, hafıf ve orta grup olup, sistemik bir ilaç kullanmaması istenmiştir. "Flow cytometry" ile CD2, CD3, CD4, CD8, CD19, CD56, CD23, CD25, CD40, CD45RA, CD45RO, HLA-DR belirleyicilerine bakılmıştır. Anova testi ile farklı olan grup değerlendirilmiştir.
CD3 ve CD4'ün sağlıklı gruba göre p<0.01 istatistiksel anlamlılık ile allerjik ve allerjik olmayan gruplarda birbirine paralel olarak arttığı gözlenmiştir. Bu bulgu stabil dönemde de astma olgularında total ve helper T lenfositlerin aktif olduğunun işaretidir. Allerjik olmayan grupta diğer gruplara göre CD2, CD45RO, HLA-DR p<0.05 istatistiksel anlamlılıkla farklı bulunmuştur. Bu durum allerjik olmayan astma olgularında hastalık seyrinin daha ciddi olabileceğini düşündürmektedir. Allerjik grupta ise CD19, CD40 p<0.01 ve p<0.001 anlamlılıkla diğer gruplardan artmış bulunmuştur. Bu allerjik grupta stabil dönemde de B lenfosit aktivitesinin artmış olduğunun göstergesidir.
Stabil dönemde allerjik ve allerjik olmayan astma olgularının periferik kan lenfosit altgrup ve aktivasyon belirleyicilerinde farklı artışlar olduğu ve astmada gelecek çalışmalarda ayrıntılı olgu seçimlerinin gerektiği sonucuna varılmıştır.
