SARKOİDOZ HASTALIĞINDA ANTİFOSFOLİPİD ANTİKORLARININ ÖNEMİ PRELİMİNER BİR ÇALIŞMA

It is well-known that patients with sarcoidosis have humoral immunologic abnormalities. Increases in serum immunglobulins and some autoantibodies as antinuclear antibody and rheumatoid factor have been reported. In a wide variety of autoimmune and non-autoimmune disorders, occurance of antiphosholipid antibodies have been detected. Our purpose was to evaluate the relationship between the occurence of antiphospholipid antibodies (APL-Ab) and sarcoidosis. IgG and IgM APL-Ab were estimated by enzyme linked immunoabsorbent assay (ELISA) in 26 untreated sarcoidosis patients (with a mean age of 45.2±16.8) whose diagnosis were established with histopathological and clinicoradiologic findings as well as 12 healthy controls (with a mean age of 45±15.7). Results.- No significant correlation was observed between serum ALP-Ab level and sarcoidosis but only 4 of the 26 sarcoidosis patients who had skin lesions and eritema nodosum showed either pozitive IgG or pozitive IgM APL-Ab. Conclusion.- This preliminary study suggets that further studies are needed to assess the relationship between skin lesions and APL-Ab levels in patients with sarcoidosis.

Sarkoidoz hastalığında humoral immunolojik bozuklukların olduğu bilinmektedir. Nitekim, serum immünglobulinlerinde ve antinükleer antikor, romatoid faktör gibi otoantikorlarda artış birçok yayında bildirilmiştir. Birçok otoimmun ve otoimmün olmayan hastalıkta antifosfolipid düzeyinde artış tespit edilmiştir. Bu nedenle, çalışmamızda sarkoidoz hastalığı ile antifosfolipid antikorları arasındaki ilişkiyi incelemek amaçlandı. Sarkoidoz tanısı histopatolojik ve kliniko-radyolojik bulgularla konmuş 26 sarkoidoz hastası (yaş ortalamalan; 45.2±16.8) ile 12 sağlıklı kontrolde (yaş ortalamaları, 45±15.7) enzim bağlı immunabsorban deneyi (Elisa) ile IgG ve IgM antifosfolipid antikor düzeyleri saptandı. Antifosfolipid antikor düzeyleri ile sarkoidoz hastalığı arasında anlamlı derecede bir ilişki saptanmadı. Ancak cilt lezyonları ve eritema nodosumu bulunan 26 sarkoidoz hastasının 4'ünde IgG ve/veya IgM antifosfolipid antikor düzeyleri yüksek bulundu. Bu preliminer çalışma, sarkoidozda cilt tutulumu bulunan olgularla, antifosfolipid antikor pozitifliği arasındaki ilişkinin aydınlatılması için geniş çapta çalışmalara gereksinim olduğunu düşündürtmektedir.
