There are several reports suggesting an association between certain human leukocyte antigents (HLA) and sarcoidosis. In different races, different HLA antigens were found more frequent in sarcoidosis patients compared to the controls. Results and Conclusion.- Thirty eight sarcoidosis patients with pulmonary and extrapulmonary involvement (31 female and 7 male, mean age 38±16.1) were included to the study. PA chest-x ray examinations revealed that 13 patients had radiological stage I, 21 stage II and 4 stage III disease. An historical control group consisting of 160 kidney donor candidates was used as control group. HLA-A, HLA-B, HLA-DL and HLA-DQ antigens were determined by lymphocytotoxicity assay. The frequencies of HLA-A2, -B8, -B22, -B38, -DR4 and DR14 were significantly higher in sarcoidosis patients (OR values 3.36, 5.42; 14.8, 29.81, 5.35 and 14.81, respectively) compared to the control group. There were no associations between radiological stage and time of onset of the disease, therapeutic response to corticosteroids, but the frequency of HLA-A2 in patients with pulmonary involvement and HLA-B38 in patients with extrapulmonary involvement was higher.

Nedeni bilinmeyen multisistemik granulomatöz bir hastalık olan sarkoidoz ile HLA antijenleri arasında ilişki olduğunu bildiren birçok yayın vardır. Farklı ırklarda farklı HLA antijenleri sarkoidozlu hastalarda, kontrollere göre daha sık bulunmuştur. Bu nedenle biz de Türk sarkoidozlu hastalarda sarkoidozda genetik yatkınlığa yol açabilecek doku antijenlerini araştırdık. Bu amaçla torasik ve ekstratorasik tutulumu olan 38 (31'i kadın, 7'si erkek, yaş ortalaması 38±16.1) hasta çalışmaya alındı. Kontrol grubu olarak Cerrahpaşa Tıp Fakültesi Kan Merkezi'ne böbrek donörü adayı olarak başvuran ve birbirleri akraba olmayan 160 kişilik bir grup kullanıldı. HLA antijenleri konvansiyonel lenfosititoksisite deneyi ile tayin edildi. İstatistiksel analizler ki-kare, düzeltilmiş p değeri ve Fisher'in kesin olasılık testi ile yapıldı. Ayrıca göreceli risk oranları (OR) hesaplandı. Hastalığın ortay¦a çıkış yaşı, torasik ve ekstratorasik tutulum, radyolojik evre, tedaviye yanıt gibi parametrelerle HLA antijenleri arasında ilişki olup olmadığı araştırıldı. Sonuç olarak HLA A2, HLA B8, HLA B22, HLA B38, HLA DR4, HLA DRl4 antijenlerinin sıklıkları sarkoidozlu hasta grubunda anlamlı derecede yüksek olarak bulundu (OR değerleri sırasıyla 3.36, 5.4?,14.81, 29.81, 5.35,14.81). Hastalığa ilişkin parametreler arasında sadece HLA A2 ile torasik tutulum ve HLA B38 ile ekstratorasik tutulum arasında anlamlı bir bağıntı saptandı.
