Antral Gastritlerde Helikobakter Pilori Yoğunluğu ve Mast Hücre Sayısı Arasındaki İlişki

Objectives: Helicobacter pylori (HP) has an important role in various gastroduodenal diseases, especially in active and chronic gastritis and mast cells is known as primary trigger of inflammation. Due to that, the relationship between the degree of HP positivity and negativity with the intensity of mast cell in the cases of antral gastritis was invested. Materials and Methods: 160 cases that were reported as antral gastritis by Fırat Ünivercity Medicine Faculty Pathology Laboratory but not diagnosed as duodenal ulcer via endoscopic examination previously, were included in this study. The study group was divided in to 4 subgroup as HP(-), HP 1+, HP 2+ and HP 3+ and each group has 40 cases. In this study, the count of mast cell in these subgroups was compared. Results: Mast cell count in antral gastritis in HP(-), HP 1+, HP 2+, HP 3+ cases was found (63,92±10,02, 86,27±11,08, 92,75±13,59, 121±17,73) respectively. The difference between HP(-) with HP 1+ cases and HP 2+ with HP 3+ cases about mast cell count was significant (p<0,001). But the difference between HP 1+ and HP 2+ cases wasn't significant (p>0,05). Conclusion: Mast cell intensity that plays important role in inflammatory process, increase with HP positivity significantly. Between HP(-) and HP(+) cases there was expressive difference and degree of HP positivity and number of mast cells there was an evident increase. Amaç: Helikobakter Pilori (HP), aktif ve kronik gastritler başta olmak üzere çeşitli gastroduodenal hastalıkların etiyolojisinde önemli rol oynar. Mast hücrelerinin de inflamasyonun primer tetikleyicisi olduğu bilinmektedir. Bu nedenle antral gastritli olgularda HP negatifliği ve HP pozitiflik derecesi ile mast hücre yoğunluğu arasındaki ilişki araştırıldı. Gereç ve Yöntem: Fırat Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı Laboratuarında antral gastrit tanısı alan ve endoskopik muayenesinde duodenal ülser bulunmayan 160 olgu çalışmaya alındı. Her birinden 40 olmak üzere HP(-), HP1+, HP 2+ ve HP 3+ antral gastritli olgularda mast hücre sayıları karşılaştırıldı. Bulgular: Mast hücre sayısı antral gastritlerde HP(-) olgularda 63.92±10.02, HP 1+ olgularda 86.27±11.0.8. HP 2+ olgularda 92.75±13.59. HP 3+ olgularda 121±17.73 bulundu. HP(-) olgularla HP1+ ve HP 2+ olgularla HP 3+ olgular arasında mast hücre sayısı arasında istatistiksel olarak anlamlı farklılık (p<0,001) bulundu. HP 1+ ve HP2+ olgular arasında ise istatistiksel olarak anlamlı farklılık görülmedi (p>0,05). Sonuç: İltihabi proçeste önemli rol oynayan mast hücrelerinin yoğunluğu, HP pozitifliği ile belirgin olarak artmaktadır. Özellikle HP(-) ve HP1+ olgular arasında mast hücre sayısında anlamlı bir farklılık ve HP pozitiflik derecesi ile mast hücre sayılarında belirgin artış bulunması, antral gastritlerde HP pozitiflik derecesiyle mast hücre artışının ilişkili olduğunu düşündürmektedir. ©2006, Fırat Üniversitesi, Tıp Fakültesi
