Methods of sphincter preservation were developed about a century ago. Combination of these techniques with adequate anterior resection permits successful utilisation of the sphincter preserving procedures. Although Miles' abdominoperineal resection (APR) still remains a golden standard for the low rectal carcinomas, restorative resection may now be possible with equivalent oncologic control of the disease and sun'ival rates. For this reason, current trends also suggest the sphincter-saving procedures to be of choice whereas APR is being less frequently being utilised. In this article anorectal anatomy and the sphincter-saving procedures utilised in modern surgery is reviewed. /.Journal of Turgut Özal Medical Center 1(4):334-341.1994/

SJinkler koruyucu ameliyatların (SKO) geçmişi bir asır önceye kadar uzanmaktadır. Hu yöntemlerin, anterior rezeksiyonla kombine olarak kullanılmaları sonucu çeşitli seviyelerdeki rektum karsinomları başarıyla tedavi edilebilir hale gelmiştir. Her ne kadar Miles'in abdominoperineal rezeksiyon (APR) yöntemi aşağı rektum karsinomalarının tedavisinde altın standard olma özelliğini korumakta ise de." sfînkter koruyucu operasyonlarla da günümüzde aynı derecede onkolo/ik olarak hastalığın kontrolü ve sağkalım oranları sağlanabilmektedir. Bu nedenle de günümüzde APR giderek daha az kullanılan bir yöntem olurken, sfînkter koruyucu operasyonlar iyi fonksiyonel sonuçları dolayısı ile öncelikle tercih edilir olmuşlardır.
