The importance of specific IgE investigation on diagnosis of allergic diseases

When suspecting an allergic disease, it should be confirmed using special tests. In this study, 79 patients who were followed up by outpatients of Allergy of Dr. Behçet Uz Children's Hospital was examined. Twenty healthy children served as the control group. We aimed to determine the importance of skin tests by comparing specific IgE and total IgE levels by EIA of patients having allergic complaints. The 59% (47) of thepatientswas male and 41 %(32) female. Average age was 107.55 ±27.82 months.The 65%(13) of the control group was male, 35%(7) female and the average age was 108.5 ±24.5 months.Patients were dispersed in a broad range diagnosis spectrum from Asthma Bronchiale (AB) to AB+allergic rinit, urticaria, allergic conjunctivitis. Levels of total and specific IgE were determined, scratch and intradermal skin tests were performed to the patients. Total and specific IgE levels of the control group were studied. In laboratory findings which were made for supporting the diagnosis, 59% was positive for total IgE, 52% and 85% was positive for scratch and intradermal skin tests respectively. Specific IgE levels were found 71% positive. In our study, while the sensitivity and specifity of total IgE were found 58.2% and 333% respectively, for specific IgE, they were found 80.5% and 83.3%. Since specific IgE which was determined by ELISA method was found highly specific and effective, we can easily claim that it is a reliable diagnostic technique which can be used in the diagnosis ofallergic diseases.

Allerjik hastalık düşünüldüğünde tanının bazı özel testler kullanılarak kesinleştirilmesi gerekmektedir. Çalışmada İzmir Dr. Behçet Uz Çocuk Hastanesi Allerji polikliniğince izlenen 79 hasta ve kontrol grubu olarak 20 sağlıklı çocuk incelendi. Bu çalışmada allerjik yakınması olan hastalarda EIA (Enzim immünoassay) yöntemi ile ölçülen spesifik Ig E düzeyini, total Ig E düzeyi ve deri testleri ile karşılaştırarak tanıdaki önemini saptamayı amaçladık. Hasta grubunun 47(%59)'si erkek, 32(%41)'si kız olup, yaş ortalaması 107.55 ±27.82 ay olarak bulundu. Kontrol grubunun ise 13(%65)'ü erkek, 7(%35)'si kız olup, ortalama yaşları 108.5 ±24.5 ay idi. Hastalar, Astma Bronşiyale (AB), AB+allerjik rinit, ürtiker, allerjik konjunktivit tanıları almışlardı. Hasta grubuna scratch ve intradermal deri testi uygulandı, ayrıca total ve spesifik IgE düzeyle i beli lendi. Kont ol g ubunun da total ve spesifik IgE düzeyle i beli lendi. Total ve spesifik IgE düzeyle i ELİSA yöntemi ile beli lendi. Hasta grubunda tanıyı desteklemek amacıyla yapılan laboratuvar incelemelerinde serum total IgE düzeyle i %59 o anında yüksek bulunu ken sc atch de i testi %52 o anında int ade mal de i testi %85 o anında olumlu saptanmıştı . Spesifik IgE düzeyi ise %71 oranında yüksek bulunmuştur. Çalışmamızda intradermal deri testleri ile karşılaştırıldığında total IgE sensitivitesi %58.2, spesivitesi %333 gibi düşük o anla da bulunu ken spesifik IgE için sensitivite %80.5 spesivite %83.3 ola ak bulunmuştu . ELİSA yöntemi ile düzeyini saptamış olduğumuz spesifik IgE'nin yüksek sensitivite ve etkinliğinin bulunması nedeniyle alle jik hastalıkla ın tanısında kullanılabilecekgüvenili bi tanıyöntemiolduğunusöyleyebili iz.
