Lomber Disk Hernilerinde Lezyon Özellikleri Ve Semptomatoloji ilişkisi

Objectives: The purpose of this study was to evaluate the relations his between the features of herniated discs in the patients with lumbar syndrome and the symptomatology which may help the clinician in making a decision about atypical cases. Methods: The files of 1083 operations scanned retrospectively. The data of the patients were evaluated statistically. Results: 979 of the 1083 were first operation for the patients. There were 424 females (43%) and 555 males (57 %). The average age of the patients was 42 years. The most frequent disc level was L4-L5 (62%). The proportion of L4-L5 discopathies was 62%; L5-S1 discopathies was 30%. Contained herniation was 62%. When neurogenic claudication exists, the proportions of disc herniations accompanying lumbar spinal stenosis and high-level lumbar disc herniations were higher. When unilateral leg pain exists, the proportions of lumbar spinal stenosis, high-level lumbar lesions, multi level lesions and bilateral-median lesions were low. Conclusion: Age, sex and symptom features of the patient can provide us some information about localization, level and accompanying pathologies of disc herniation.

Amaç: Bu çalışmada amaç lomber sendromlu hastalarda herniye disklerin özellikleri ile hastaların prezantasyonu arasındaki ilişkileri inceleyerek özellikle atipik vakalar için klinisyenin hasta hakkında karar vermesine yardımcı olabilmekti. Yöntem: 1083 lomber disk operasyonunun dosyaları retrospektif olarak tarandı. Hastaların verileri istatistiksel olarak değerlendirildi. Bulgular: 1083 operasyonun 979'u ilk operasyon olup 424'ü kadın, 555'i erkekdi. Yaş ortalaması 42 idi. En sık rastlanan disk seviyesi L4-L5 (%62) idi. Contained herniasyon %54 oranında saptandı. Semptomlar içinde Nörojenik Kladikasyo varsa, lomber disk hernisine eşlik eden lomber dar kanal ve üst seviye lomber disk herniasyonu saptanması artıyordu. Tek taraflı bacak ağrısı varsa, lomber dar kanal, üst lomber lezyon, bilateral-orta hat lezyonu ve multiseviye lezyon saptanma oranları azalıyordu. Sonuç: Hastanın yaş, cins ve semptom özellikleri, lomber disk herniasyonunun lokalizasyonu, seviyesi ve eşlik eden patolojiler hakkında bize bilgi sağlayabilmektedir.
