The relationship between the tumor angiogenesis, pathologic stage and the tumor grade in prostate carcinoma

ween the tumor angiogenesis, pathologic stage and the tumor grade in prostate carcinoma. In this rescarch, the correlation belween the tumor mierovessel density and the parameters of the pathologic stage and grade of the prostate carcinoma was İnvestiga-ted on 63 radical prostateetomy materials examined and reported in our department. The tumo-ral microvessels vvere immunohistochemically highlighted using anti-CD34 antibody, and co-unted in 5 high power fİeld in which they were most intense. We determined the angiogenesis index (Aİ) by taken their mean values. The mean Aİ value was 58.4+20.1 in tumors wİth cap-sule invasion (n=47), and 52.3+29.3 in tumors without capsuîe invasion (n=16) (p-0.02). In 15 cases with seminal vesİcie invasion, the mean Aİ value was 66.1+18.5 vvhereas İn 48 cases wİthout invasion the mean was 54.0+23.5 (p=0.048). We conelude that there is a significant relation betvveen the angiogenesis and the capsule İnvasion of the tumor. We found a quİet sig-nificance betvveen the Aİ and the seminal vesicle invasion. The both surgical margİn positivity and the lymph node positivity vvere found to be independent from the Aİ. There vvas no correlation betvveen the mean Aİ values of the group vvhich had a Gleason score of 6 and under and that of the group having a Gleason score of 7 and above. It has been rcvealed that the mean Aİ value correlated wifh the tumor grade and the tumor stage İn various researeh. Hovvever there arc others reporting conflicting rcsults on the samc ground. On the other hand, many authors have found that there vvas a correlation betvveen the Aİ and tumor progression after radical prostateetomy. This preliminary study İs being follovved by a nevv one in vvhich the relations-hip betvveen the Aİ and the survival of the same patient popıılation is further investigated.

Bu çalışmada Anabilim Dalımızda incelenmiş ve random olarak seçilmiş 63 radikal prostatek-tomİ materyalinde tümör mikrodamar yoğunluğunun patolojik evre parametreleri ve grade ile korelasyonu araştırıldı. İmmünohistokimyasal yöntemle anti-CD34 antikoru kullanılarak işaretlenen mİkrodamarlar cn sık oldukları bölgede 5 büyük büyütme alanında sayılıp, ortalama değer alınarak anjiogenez indeksi (Aİ) belirlendi. Kapsül invazyonu görülen vakalarda (n-47) Aî ortalaması 58.4+20.01, kapsül negatif vakalarda (n=16) 52.3±29.9 olarak belirlendi (p=0.02). Vezikula seminalis invazyonu gösteren 15 vakada 66.1+18.5 olarak saptanan Aİ ortalaması, invazyon olmayan 48 vakada 54.0+23.5 olarak saptandı (p=0.048). Sonuç olarak Aİ ile kapsül invazyonu arasında anlamlı bir ilişki olduğu, vezikula seminalis invazyonu ile Aİ arasındaki korelasyonun ise sımrda anlamlı olduğu belirlendi. Diğer taraftan cerrahi sınır ve lenf ganglionu pozitivitesinin Aİ'dcn bağımsız olduğu sonucuna varıldı. Gîcason skoru 6 ve altında olan grup ile 7 ve üstünde olan gıubun anjiogenetik aktİviteleri arasında korelasyon saptanmadı. Değişik çalışmalarda evre ve grade ile Aİ arasında ilişki saptanırken, bu yönde çelişkili sonuçlar elde eden çalışmalar da vardır. Birçok çalışmada ise Aİ ile radikal prostatektomi sonrası progresyon arasında korelasyon saptanmıştır. Bu grubu içeren hastaların prognozlan ile İlişki bu ön çalışmanın devamı niteliğinde sürdürül inektedir.
