Microalbıtminuıia and QT dispersion in patients with newly diagnosed systeınic hypertension. Microalbuminuria (MA) and left ventricular hypertrophy (LVH) are associated with inere-ased cardiovascular morbidity and mortality in hypcrtensive patients. It has been shown to be inereased QT dispersion in hypertension. The aim of our study was to examine if there was any association bervveen MA and QT dispersion in hypertensive patients. Eighty-one consecu-tive newly diagnosed hypertensive patients and 33 healthy controls (15 female, 18 male, mean age: 46.2±11.1 yr) were enrolled in thc study. Patients with hypertension were dividcd into two groups: nine-teen patients in MA positive (MA+) group (8 female and 11 male, mean age: 5İ.9±9.3 yr) and 62 patients İn MA negative (MA-) group (29 female and 33 male, mean age: 50.2+9.8 yr). Echocardiography, 12-lcad eleetrocardiography, three times albumin analysis İn 24-hours urine and rutin biochemical tests were performed to ali the patients. Corrected QT (QTc) dispersion in MA+ (62.1±23.9 ms) and in MA- (55.8+19 m) groups was sİgnİficantly highcr than controls (29.2+10.6 ms) (p<0,001). The prevalence of LVH was also significantly higher in MA+ (6/19, 31.6%) and MA- groups (16/62, 25.8%) than controls (2/33, 6%) (p<0.05). There was no difference between MA+ and MA- groups with respect to LVH and QTc dispersion (p>0.05). In ali the groups, there was a significant correlation only between QTc dispersion and systolİc blood pressure in MA+ group (r=0.648, p=0.003). QT dispersion in hypertensive population is-signifİcantly higher than healthy population. Renal and cardiac involvement may not oecur in parallcl fashion in newly diagnosed hypertensive patients.

Hipertansif hastalarda mikroalbüminüri (MA), sol ventrikül hipcrtrofisi (SVH), artmış kardi-yovaskiiler morbıdite ve mortalite ile ilişkilidir. Hipertansiyonda QT dispersiyonunun normal populasyona göre daha yüksek oranda olduğu gösterilmiştir. Bu çalışmanın amacı hipertansif hastalarda MA ile QT dispersiyonu arasında bir ilişkinin olup olmadığım araştırmaktır. Çalışmaya daha önce tedavi almamış yeni tanı konmuş ardısıra gelen 81 hipertansif hasta ve 33 (15 kadın, 18 erkek, ortalama yaş: 46.2+11.1 yıl) sağlıklı kontrol alındı. Hipertansif hastalar 19 MA pozitif (MA+) (8 kadın, 11 erkek, ortalama yaş: 51.5+9.3 yıl) ve 62 MA negatif (MA-) (29 kadın, 33 erkek, ortalama yaş: 50.2+9.8 yıl) olmak üzere İki gruba ayrıldı. Tüm hastalara 12 derivasyonlu elektrokardiyografi, ekokardiyografi, üç kez 24 saatlik idrarda MA tayini ve rutin biyokimyasal testler yapıldı. MA+ ve MA- gruplarda düzeltilmiş QT (dQT) dispersiyonu (sırasıyla 62.1+23.9 ms ve 55.8+19 ms) kontrollere (29.2+10.6 ms) göre anlamlı olarak daha yüksek bulundu (p<0,001). SVH, MA+ (6/19, %31.6) ve MA- (16/62, %25.8) gruplarda yine kontrollere (2/33, %6) göre daha yüksek orandaydı (p<0.05). MA+ ile MA- gruplar arasında dQT dispersiyonu ve SVH açısından anlamlı bir fark saptanmadı (p>0.05). Tüm gruplarda dQT dispersiyonu sadece MA+ grubundaki sistolik kan basıncı ile anlamlı bir korrelasyon göstermekteydi (r=0.648, p^0.003). Yeni tanı konulan hipertansif hastalarda QT dispersiyon sıklığı normal populasyondan daha fazladır. Hipertansif grupta MA daha sık görülmekle birlikte bu hastalarda renal ve kardiyak tutulum paralel seyretmeyebilir.
