MULTIDISCIPLINARY APPROACH IN A CASE WITH TALON CUSPED MESIODENS AND AN IMPACTED CENTRAL INCISOR

Classical approach in malocclusion due to supernumerary teeth consists of the extraction of supernumeraries and the rehabilitation of the permanent teeth. Extra tuberculcs of the supernumerary teeth is a rare condition, and it is more uncommon to see this tubcrcule on the vestibular surface. Surgical and prosthetic approach is planned in this case, because the mesiodens has erupted, and it resembled the impacted permanent incisor; so because of the possibility of an unsuccessful orthodontic treatment, surgical and prosthetic treatment is compulsively planned. Left upper central incisor is impacted and a large mesiodens with a prominent talon cusp on the vestibular surface has erupted at the place of the incisor. After the surgical extraction of the impacted incisor, a composite laminate restoration was applied on the mesiodens talon with minimum removal at the enamel level of the talon cusp. Treatment planning can be made due to the clinical findings of the patients. The most important criteria is to handle the treatment planning of the prosthetic rehabilitation of the patient with the minimum harm and - as in our case - appropriately using the supernumerary teeth.

Arlık dişlerin neden olduğu gömük dişler ve bunlara bağlı malokluzyonun klasik tedavisi artık dişlerin çekimi ve daimi dişlerin rehabilitasyonudur. Artık dişlerde fazla tüberkül görülmesi çok nadir bir olaydır. Bu tüberkülün vestibülde görülmesi ise daha seyrektir. Ancak bu olguda meziödensin sürmüş olması, bu nedenle gömük kalan daimi dişe benzeyen bir morfolojisinin bulunması; başarısız bir ortodontik tedavi ihtimali nedeni ile cerrahi ve protetik yaklaşımı gerektirmiştir. Sol üst orta kesici dişi gömük olan bu vakada, dişin sürmesi gereken yerde oldukça geniş çaplı ve vestibülünde belirgin bir talon tüberkülü bulunan meziodens saptanmıştır. Gömük dişin cerrahi olarak çekiminin ardından, meziodens dişin talon tüberkülü mine seviyesinde aşındırılarak, kompozit laminate restorasyonu uygulanmıştır. Tedavi planlaması her hastanın klinik bulgularına göre değişik şekillerde yapılabilir. Buradaki en önemli kriter, hastanın protetik rehabilitasyonunun hastaya en a/ zarar verecek şekilde planlanması ve olgumuzda olduğu gibi uygun şekilde ve konumda bulunan artık dişlerden de fayda] anılabilmesidir.
