Purpose: The aim of this study was to detect the effectiveness of mannitol's antioxidant capacity in the treatment of İschemia-reperfusion İnjury after acute peripheric aderial occlusion. Material and Methods: 20 healthy individuals and 32 patients with acute artehal occlusion were taken to study. Of the acute ischemia diagnosed patients, standard surgery was performed on 17 patients and surgery and mannitol infusion were given to 15 paîients. Blood samples were taken from the deep veins and MDA, CPK, GSH-PX levels were measured in the acute ischemia diagnosed period and 120th minutes of the reperfusion. Results: MDA of mannitol treatment group in the ischemic period meaningfully İncreased in consideration with the control group. İn consideration of repedusion period with ischemic group in mannitol treatment MDA signjficantly decreased. İn the reperfusion period of mannitol treatment GSH-PX significantly İncreased according to control group. Although the CPK results showed significant increase in each period of both treatments, the reperfusion period of mannitol treatment showed a significant decrease in CPK considering with it's İschemic period. Conclusion: İn acute arterial occlusions it was shown that mannitol decreases the activity of free oxygen radicals, increases the antioxidant capacity, and decreases the tissue injury meaningfully.

Amaç: Akut periferik arîeriyel tıkanmayı takiben gelişen iskemi reperfüzyon hasarının tedavisinde mannitoiün antioksidan kapasitedeki rolünün ortaya konulması amaçlandı. Gereç ve Yöntem: Akut İskeminin saptandığı 32 hasta çalışmaya alındı. 20 sağlıklı birey kontrol grubunu oluşturdu. Hasta grubunun 17' sine standart cerrahi tedavi uygulandı, 15'ine cerrahi tedavi ile beraber mannitol İnfüzyonu yapıldı. İskemik dönemde ve reperfüzyonun 120. dakikasında derin venden alınan kan örneklerinde MDA, GSH-PX ve CPK çalışıldı. Bulgular: Mannitol tedavi grubunun iskemi dönemi. MDA değeri; kontrol grubuna göre önemli ölçüde yüksekti. MDA değeri mannitollü reperfüzyon döneminde daha düşüktü. Mannitol tedavi grubunun reperfüzyon dönemindeki GSH-PX değeri kontrol grubuna göre daha yüksekti. CPK düzeyleri her iki tedavi protokolünün her döneminde kontrole göre önemli yükseliş sergilerken; mannitol tedavisinin reperfüzyon dönemi kendi iskemik dönemine göre önemli azalma ortaya koydu. Sonuç: Mannitoiün serbest oksijen radikal akitivitesini baskılayıp, antioksidan kapasiteyi artırdığı ve doku hasarında anlamlı düşme meydana getirdiği ortaya kondu.
