ERİTROPOETİNİN KEMOTERAPİ UYGULANAN RATLARDA KOLON ANASTOMOZLARININ İYİLEŞMESİ ÜZERİNE ETKİSİ

Aim: We aimed to search the effects of erythropoietin on anastamoses recovery in the rats to which chemotherapy was applied and intestine anastomos was made. Material and Method: The study was carried out at Selcuk University Experimental Research Center (SUDAM). The rats were divided into 4 groups including 10 subjects. The subjects in all groups were weighed before the surgical operation and laparotomy was made with middle line cutting. Nothing but standard feeding was made to the subjects in the 1st group after making sigmoid colon anastomoses. Subcutan erythro¬poietin injection was administered to the 2nd group after colon anastomoses. The 3rd group was injected intraperitoneal 5- fluorouracil after colon anastomoses was made. The 4th group was ma¬de intraperitoneal 5- fluorouracil after colon anastomoses, and subcutan erythropoietin injection was administered.On the 7th day following the anastomoses, the rats were weighed and they we¬re sacrificed. Anastomoses bursting pressure among the groups, hydroxyproline level in the samp¬les taken from anastomoses line and collagen and rich bind tissue formation from the fibroblast his-topathologically, the rise angiogenesis, intensity in inflammation and the existence of necrosis were evaluated. Results: In the rats to which erythropoietin was applied, the bursting pressures and hydroxyproline levels were significantly higher than those of the control group statistically. During the histopathological examination, it was seen that rich bind tissue formation from angiogenesis and collagen in the erythropoietin group rose while there was a significant reduction in the formation of inflammation and necrosis. These positive effects of erythropoietin was also established in the rats to which 5- fluorouracil was applied. Conclusion: In the rats to which colo-colic anastomoses was made, it was considered that erythropoietin inflammation had positive effects on angiogenesis with its influences on proliferation and restructuring. The same positive effects were observed in the rats which used 5-fluorouracil. Thus, it can be expected that erythropoietin can reduce the risk of anas-tomoses escape after colo-colic anastomoses and in conditions such as chemoteraphy, which ne-gatively affects the recovery of anastomoses.

Amaç: Eritropoetinin barsak anastomozu yapılan ve kemoterapi uygulanan ratlarda anastomoz iyi-leşmesi üzerindeki etkilerini araştırmayı amaçladık. Gereç ve Yöntem: Çalışma Selçuk Üniversitesi Deneysel Araştırma Merkezi (SÜDAM)'inde gerçekleştirildi. Ratlar 10'ar denekten oluşan 4 gruba ay-rıldı. Bütün gruptaki denekler cerrahi işlem öncesi tartıldı ve orta hat kesisi ile laparotomi yapıldı. I. grup deneklere sigmoid kolon anastomozu yapıldıktan sonra standart besleme dışında bir işlem ya-pılmadı. II. gruba kolon anastomozu sonrası subkutan eritropoetin (EPO) injeksiyonu yapıldı. III. gru¬ba kolon anastomozu yapıldıktan sonra intraperitoneal 5-fluorourasil (5-FU) injekte edildi. IV. gruba kolon anastomozu sonrası intraperitoneal 5-fluorourasil ve subkutan eritropoetin injeksiyonu yapıldı. Anastomozdan sonraki 7. günde ratlar tartıldı ve sakrifiye edildi. Gruplar arasındaki anastomoz pat¬lama basıncı, anastomoz hattından alınan numunelerde hidroksiprolin seviyesi ve histopatolojik ola¬rak kollajen ve fibroblastdan zengin bağ dokusu oluşumu, anjiyojenezdeki artış, inflamasyondaki yo¬ğunluk ve nekroz varlığı değerlendirildi. Bulgular: Eritropoetin uygulanan ratlarda diğer gruplara göre patlama basınçları, hidroksiprolin düzeyleri istatistiksel olarak anlamlı derecede yüksekti. Histo¬patolojik incelemede eritropoetin grubunda anjiyojenez ve kollajenden zengin bağ dokusu oluşu¬munda artış, inflamasyon ve nekroz oluşumunda anlamlı şekilde azalma görüldü. Eritropoetinin bu olumlu etkileri 5-fluorourasil uygulanan ratlarda da tespit edildi. Sonuç: Kolokolik anastomoz yapılan ratlarda eritropoetinin inflamasyon, proliferasyon ve yeniden yapılanma üzerine olan etkisiyle anjiyo-jeneze olumlu etkiler yaptığı düşünülmüştür. Aynı olumlu etkiler 5-fluorourasil kullanılan ratlarda da gözlenmiştir. Bu nedenle eritropoetinin kolokolik anastomozlardan sonra ve özellikle kemoterapi gi¬bi anastomoz iyileşmesini olumsuz etkileyen durumlarda anastomoz kaçağı riskini azaltacağı beklene¬bilir.
