Aim: We aimed to compare in patients with hyperdynamic septic shock, the effects of epinephrine or norepinephrine on hemodynamic variables, urine output and metabolic values. Material and Method: In this study, we planned to retrospective study which compared norepinephrine and epinephrine in septic patients. Twenty-five patients with septic shock, according to the criteria of the 1992 American College of Chest Physicians/ Society of Critical Care Medicine Consensus Conference were included and norepinephrine or epinephrine were replaced to patients with severe septic shock in whom dopamine or fluid replacement was insufficient to achieve a MAP>60 mmHg. Hemodynamic parameters , arteriel gase analysis, urine output were analysed.Results: Hemodynamic variables were similar between two groups. Levels of lactate and glucose were higher and urine output and pH were lower, with epinephrine than with norepinephrine. Conclusion: Norepineprine and epinephrine have similar hemodynamic effects, but epinephrine can impair metabolic distrubance and urine output

Amaç: Hiperdinamik septik şoklu hastalarda, epinefrin veya norepinefrin kullanımının hemodinamik değişiklikler, idrar çıkışı ve metabolik değerlere etkisini karşılaştırmayı amaçladık. Gereç ve Yöntem: Bu çalışmada septik hastalarda norepinefrin ve epinefrin kullanımını karşılaştıran retrospektif bir çalışma planladık. 1992 American College of Chest Physicians/ Society of Critical Care Medicine Consensus toplantısına göre septik şok tanısı alan 25 hasta dahil edildi ve dopamin ve sıvı replasmanı ile ortalama arter basınçları (OAB) 60 mmHg üzerinde tutulamayan bu hastalara norepinefrin veya epinefrin verildi. Hemodinamik parametreler, arteriyel kan gazı analizi ve idrar çıkışı değerlendirildi. Bulgular: Hemodinamik veriler iki grup arasında benzerdi (p>0.05). Epinefrin grubunda, norepine-frine göre laktat ve glukoz seviyeleri daha yüksek, idrar çıkışı ve pH daha düşüktü (p<0.05). Sonuç: Norepinefrin ve epinefrin benzer hemodinamik etkilere sahip olmakla birlikte epinefrin metabolik dengede ve idrar çıkışında bozulmaya neden olabilir.
