The aim of this study was to evaluatle of adnexal masses cases that were diagnosed and laparoscoplcally treated during pregnancy in our clinic. This retrospective study included 18 cases that were diagnosed to have adnexal masses during pregnancy or referred from other clinics to our clinic and treated by laparoscopy between January 2005 and December 2009. Patient's characteristics such as age, gravidity, parity, symptoms, gestational age at the time of diagnosis, sonographic findings, and type of laparoscopic surgery, operation time and hospitalization time were recorded. Eighteen women with adnexal masses during pregnancy who underwent laparoscopic surgery were included in this study. The average age of the patients was 28.5±4.3 (range 20-37) years and the average gestational age was 13.1±3.3 (range 8-20) weeks. The patients presented usually with pelvic pain. Adnexal cyst was mostly seen by ultrasonographic evaluation. Adnexal torsion was detected in 8 patients. Nine patients were in first trimester, other 9 patients were in second trimester. We performed laparoscopic cystectomy in 10 patients, detorsion and cystectomy in 6 patients and salpingo-ooferectomy in 2 patient's. Mean adnexal mass size was 7.8±2.3 (range 5-15) cm. Mean operation time was 37.2±7.1 (25-50) minutes and mean hospitalization time was 1.8±0.6 (1-3) days. Abortion was not detected in our cases. According to these findings, laparoscopic surgery is the most appropriate treatment for patients with adnexal masses during pregnancy in the presence of expert surgeons. Laparoscopic surgery if performed by experienced surgeons seems to be safe and better management method for maternal-fetal health.

Bu çalışmada amacımız kliniğimizde gebelik esnasında adneksiyal kitle saptanan ve laparoskopik cerrahi yaklaşımla tedavi edilen olgularımızın değerlendirilmesidir. Ocak 2005 ile Aralık 2009 yılları arasında, kliniğimizde gebelik esnasında adneksiyal kitle tespit edilen veya dış merkezlerden kliniğimize gebelik+adneksiyal kitle tanısıyla sevk edilen ve laparoskopik yaklaşımla tedavisi yapılan 18 olgunun verileri retrospektif olarak incelendi. Tüm olgular yaş, gravida, parite, başvuru şikâyetleri, başvuru anındaki gebelik haftaları, ultrasonografi (USG) bulguları, uygulanan laparoskopik yöntemler ile operasyon ve hospitalizasyon süreleri açısından incelendi. Çalışmamız 18 olgudan oluşmaktaydı. Hastaların ortalama yaşı 28.5±4.3 (20-37) ve ortalama gebelik haftaları 13.1±3.3 (8-20) hafta idi. Hastalar sıklıkla kasık ağrısı şikayeti ile başvurmuşlardır. Hastalarda USG bulguları olarak gebelik ve sıklıkla adneksiyal bölgede kist tespit edilmiştir. 8 hastada aynı zamanda adneksiyal torsiyon saptanmıştır. Hastalarımızın 9'u 1. trimesterde, 9'u ise 2. trimesterde idi. Laparoskopik olarak 10 olguya kistektomi, 6 olguya detorsiyon ve kistektomi ve 2 olguya da salpingo-ooferktomi yapılmıştır. Ortalama adneksiyal kitle çapı 7.8±2.3 (5-15) cm idi. Ortalama operasyon süresi 37.2±7.1 (25¬50) dakika ve ortalama hastanede kalış süresi 1.8±0.6 (1-3) gün idi. Hiçbir olgumuzda abortus gelişmemiştir. Gebelik esnasında adneksiyal kitlelere cerrahi yaklaşımda uygun hastalarda ve deneyimli laparoskopistlerin varlığında en uygun yöntem laparoskopik cerrahi yapılmasıdır. Uzman bir kadro tarafından yapıldığında laparoskopik cerrahi anne ve bebek açısından güvenli ve avantajlı görülmektedir.
