The place of surgical treatment, except for the treatment of complications, in peptic ulcer gradually decreases. Helicobacter pylori eradication with proton pump inhibitors (PPI) and H2 receptors that have been used frequently recently have played important role in this decrease. One of the most common complications of peptic ulcer, though protective medicines, is still perforations and they are very common among surgical emergencies. The files of 311 patients with peptic ulcer perforation accepted to the clinic between 2000-2010 were examined retrospectively. The patients applied to our clinic with peptic ulcer perforation were examined in terms of age, sex, seasonal relation, and applied surgical technique. 206 of the patients were male with 52.8 mean age and 105 of them were female with 65.2 mean age. 266 of the patients were fixed with primary suture, 35 of them had bilateral truncal vagotomy+pyloroplasty and 10 were fixed laparoscopically. Average hospitalization time was 7 days. While wound infection developed in 25 of the patients, evisceration was developed in 8 patients, 9 patients were exetus. Over the last ten years there has been a change from definitive attempts to primary fixation and from laparotomy to laparoscopic methods for treatment of peptic ulcer perforations our clinic.

Peptik ülser hastalığında cerrahi tedavinin yeri komplikasyonlarının tedavisi dışında giderek azalmaktadır. Son yıllarda sıkça kullanılmakta olan H2 reseptör antagonistleri ve proton pompa inhibitörleri (PPI) ile helikobakter pilori eradikasyonu bu azalmada önemli rol oynamaktadır. Koruyucu ilaçlara rağmen peptik ülserin en sık karşılaşılan komplikasyonlarından birisi halen perforasyonlardır ve cerrahi aciller içerisinde oldukça sık rastlanır. Kliniğimize 2000-2010 tarihleri arasında ülser perforasyonu nedeni ile müracaat eden 311 hastanın dosyaları retrospektif olarak incelendi. Kliniğimize müracaat eden peptik ülser perforasyonlu hastalar yaş, cinsiyet, mevsimsel ilişki ve uygulanan cerrahi teknik açısından irdelendi. Hastaların 206'sı erkek ve yaş ortalamaları 52.8, 105'i kadın ve yaş ortalamaları 65.2 idi. Hastaların 266'sına primer sütür ile onarım, 35'ine bilateral trunkal vagotomi (BTV)+Piloroplasti ve 10'una laparoskopik onarım yapıldı. Ortalama hastanede kalış süreleri 7 gün idi. Hastaların 25'inde yara yeri enfeksiyonu, 8'inde evisserasyon gelişirken 9 hasta kaybedildi. Kliniğimizde son 10 yıl içerisinde peptik ülser perforasyonların tedavisinde definitif girişimlerden primer onarıma ve laparotomiden laparoskopik yöntemlere doğru bir seyir izlenmiştir.
