Moderate Intensity Eccentric Exercise results in Increases in Plasma CK Level without effecting any other damage indicators

Muscular damage induced by eccentric exercise is characterized by decrease in muscular strength, muscular pain and elevated levels of serum enzymes. The purpose of this study was to evaluate parameters of physiological and biochemical as indicator of muscle damage induced moderate intensity eccentric exercise. Eleven healthy male subjects (mean age: 21.45±0.93 years) participated in this study. Subjects performed 6 sets of 8 maximal eccentric exercise, the knee joint through a range of motion from 180 degrees to 90 degrees (180 degrees = full extension), at the velocity 60 degrees.s-1 on an isokinetic dynamometer. Blood samples were taken just prior, immediately after, and 2, 3, 5, and 7 days postexercise. Plasma levels of creatine kinase (CK), lactate dehydrogenase (LDH), aspartat aminotransferase (AST) and alanin aminotransferase (ALT) enzymes during seven days, platelet aggregation induced by collagen and ADP were evaluated from collected blood samples immediately after exercise. The measurement of isometric peak torques of flexors and extensors of the knee joint was performed at 90 degrees 2, 3, 5, and 7 days post-exercise. Although there was no change in isometric flexor strength immediately after and post-exercise, isometric extensor peak torque increased on days fifth and seventh (p<0.05). While level of serum CK reached the highest on 3 day post exercise (p<0.05), an increase was not observed in levels of LDH, AST and ALT (p>0.05). Pre and postexercise no difference between ADP and platelet aggregation induced by collagen was observed (p> 0.05). In this study, moderate intensity eccentric exercise did not cause a change on loss of strength physiological indicator of muscle damage and on trombosit aggregation a indicator of inflamation after muscle damage, however it has been concluded that it caused an increase at level of serum CK from biochemical parameters of muscle damage. This findings show that serum CK levels could be the most sensitive indicator of muscle damage. On the other hand by evaluating only CK levels may not reflect any changes in other muscle damage parameters measured.

Ekzentrik egzersiz kaynaklı kas hasarı kas kuvvetinde azalma, ağrı ve artmış serum enzim düzeyleri ile karakterizedir. Bu çalışmanın amacı orta şiddette yapılan ekzentrik egzersizle oluşturulan hasar belirteci olarak fizyolojik ve biyokimyasal parametreleri değerlendirmekti. On bir sağlıklı erkek denek (ortalama yaş: 21.45±0.93 yıl) çalışmaya katıldı. Deneklere izokinetik dinamometrede 60 der/sn hızda, diz eklemi 180 derece tam ekstansiyondan 90 derece fleksiyona gelecek şekilde 6 set 8 tekrar maksimal ekzentrik egzersiz yaptırıldı. Kan örnekleri, egzersizden önce, hemen sonra ve egzersizi takip eden 2., 3., 5., ve 7. günlerde alındı Kan örneklerinden serum kreatin kinaz (KK), laktat dehidrogenaz (LDH), aspartat aminotransferaz (AST) ve alanin aminotransferaz (ALT) enzimleri 7 gün boyunca, ADP ve Kollajen ile indüklenen trombosit agregasyonu ise egzersizi hemen takiben değerlendirildi. Diz ekleminin ekstansör ve fleksör izometrik pik tork ölçümleri 90 derecelik açıda egzersiz sonrası 2., 3., 5., ve 7. günlerde gerçekleştirildi. Egzersizden hemen sonra ve egzersiz sonrası günlerde izometrik fleksör kuvvette değişme olmazken izometrik ekstensör pik tork 5. ve 7. günlerde artış gösterdi (p<0.05). KK değerlerinin egzersiz sonrası 3. günde en yüksek seviyeye ulaştığı gözlenirken (p<0.05), LDH, AST ve ALT seviyelerinde değişim gözlenmedi (p>0.05). ADP ve Kollajen ile indüklenen trombosit agregasyonunun egzersiz öncesi ile egzersiz sonrası arasında fark bulunmadı (p> 0.05). Bu çalışmada orta şiddette yapılan ekzentrik egzersizin, hasarın fizyolojik göstergesi olan kuvvet kaybı ve hasar sonrası inflamasyonun bir göstergesi olan trombosit agregasyonu üzerinde bir değişime yol açmadığı, ancak hasarın biyokimyasal parametrelerinden yalnızca KK seviyesinde bir artışa neden olduğu sonucuna varılmıştır. Bu sonuçlar bir yandan hasar belirteci olarak KK’ın daha duyarlı bir parametre olabileceğini gösterirken yalnız başına KK sonuçlarını değerlendirmenin diğer belirteçlerde bir artma olduğu anlamına gelmeyeceğini de ortaya koymaktadır.
