Adnexial torsion is a rarely diagnosed cause of acute abdominal pain in pregnancy. It is frequently difficult to diagnose cases of adnexal torsion. Clinical and laboratory findings are not specific enough for diagnosis. In pregnancy, it is a very difficult situation for clinician. In this case, we presented a 21-year-old , gravida 1, parity 0, 6 weeks pregnancy with acute abdomen. Ultrasonographic finding of the patient showed a 8x7 cm cystic mass on the right adnexal area with intracystic hemorragic appearance. The patient was operated on the progression of symptoms of acute abdomen; right salpingooferektomi was performed. Ninth postoperative day, spontaneous abortion occured. The diagnosis of ovarian torsion during pregnancy is very difficult, the operation is inevitable. Corpus luteum, the source of progesterone, is taken during oophorectomy; pregnancy is more difficult due to this situation.

Adneksiyel torsiyon, gebelikte saptanan nadir akut karın nedenlerindendir. Adneksiyel torsiyon vakalarında tanı koymak oldukça güçtür. Klinik ve laboratuvar bulguları da tanı koymak için yeterince spesifik değildir. Gebelikte ise klinisyen için çok zor bir durumdur. Biz bu vakada 21 yaşında akut batın ile başvurmuş 6 haftalık, gravida 1, parite 0 olan bir vakayı sunuyoruz. Hastanın ultrasonografi bulgusunda sağ adneksiyel alanda kist içi hemoraji görünümü veren 8x7 cm’lik kistik kitle mevcuttu. Akut batın bulgularının ilerlemesi üzerine laparatomi ile operasyona alınan hastaya sağ salpingooferektomi uygulandı. Postoperatif 9. günde spontan abortus gerçekleşti. Gebelik sırasında over torsiyonu tanısı koymak oldukça güçtür, operasyon ise kaçınılmazdır. Gebeleğin devamı progesteron kaynağı olan corpus luteumun ooferektomi sırasında alınması nedeni ile güçleşmektedir.
