FLOROZİSLİ MİNEDE TAM-SERAMİK RESTORASYONLARIN MAKASLAMA BAĞLANMA DİRENÇLERİNİN DEĞERLENDİRİLMESİ

DETERMINATION OF SHEAR BOND STRENGTH OF FULL-CERAMIC RESTORATIONS ON FLUOROSED ENAMEL

Introduction: Dental fluorosis often causes esthetical problems and mainly affects the enamel. Restoring of fluorosed teeth may be necessary owing to esthetical and/or functional reasons. Therefore, ceramic restorations can be used. Objectives: The aim of this study was to determine the shear bond strength of all-ceramic restorations to mild and middle fluorosed enamel. Material and Method: Teeth were classified as "TFI=0 (control), TFI=1-3 and TFI=4-6 groups" according to Thylstrup-Fejerskov Index (TFI). Enamel surfaces were flattened in 0.5 mm depth by "Microcut- Precision Cutter/Metkon,Turkey" and etched with "Etchinggel/ 3M,USA"(35%,20s). IPS-Empress ceramic discs (Ivoclar/Vivadent,Liechtenstein) (3mmx3mm, n=10, totaling 30) were bonded to the teeth using a totaletch bonding system (Single Bond/3M,USA) and a composite luting cement (Opal Luting/3M,USA). Teeth were immersed in distilled water (at room temperature, 24h), thermocycled (+50C/+550C,x500), tested with "Universal- Testing-Machine" (Lloyd-LRX Universal/Fareham,England) (crosshead speed:1 mm/minute). Bond failures were examined under stereomicroscope (2.5x10). Data were examined with One-Way- ANOVA and LSD Test statistically p<0,05). Results: The values were decreased due to increase of levels of fluorosis. Statistical differences were present between "TFI 0 and TFI 4-6" and "TFI 1-3 and TFI 4-6" (p<0,05). It was not found between "TFI 0 and TFI 1-3" (p>0,05). The type of bond failure was mostly "adhesive" between intersurfaces of tooth/restoration. "Cohesive" failures were not found in ceramic discs. Conclusions: Shear bond strength has shown a decreasing due to the severity of fluorosis. Bond failure was commonly adhesive. Allceramic restorations may safely be used for fluorosed enamel in TFI 1-3, but they may cause bonding problems in TFI 4-6

Giriş: Dental fluorozis sıklıkla dişlerde estetik problemlere yol açmakta ve esas olarak mineyi etkilemektedir. Estetik ve/veya fonksiyonel nedenlerle florozisli dişlerin restorasyonları gerekebilir ve bu amaçla seramik restorasyonlar kullanılabilir. Amaç: Bu çalışmanın amacı; hafif ve orta şiddetteki florozisli daimi dişlerin minesinde, tam-seramik restorasyonların makaslama bağlanma dirençlerinin değerlendirilmesidir. Gereç ve Yöntem: Dişler, Thylstrup-Fejerskov Indeksi'ne (TFI) göre; TFI=0 (kontrol), TFI=1-3 ve TFI=4-6 gruplarına ayrıldı. Mine yüzeyleri, kesme cihazıyla (Microcut-Precision Cutter/Metkon,Türkiye) 0,5mm düzleştirildi ve %35'lik fosforik asitle (Etching-gel/3M,USA) 20sn dağlandı. IPS-Empress seramik diskleri (Ivoclar/Vivadent,Liechtenstein) (3mmx3mm, n=10, toplam=30 örnek), bir total-etch bonding sistem (Single Bond/3M,USA) ve bir kompozit yapıştırma simanı (Opal Luting/3M,USA) kullanılarak mine yüzeylerine yapıştırıldı. Dişler, oda sıcaklığında 24 saat, distile suda bekletildi. +5 C/+550C 'de 500 kez termosiklus uygulandı ve lmm/dk'lık başlık hızına sahip bir "Universal Test Cihazı" (Lloyd- LRX Universal/ Fareham, England) ile makaslama bağlanma direnci testine tabi tutuldu. Bağlanma başarısızlığı açısından, mine yüzeyleri stereomikroskopla incelendi (2.5x10).Verilerin istatistiksel değerlendirilmesinde Tek-Yönlü-ANOVA ve LSD Testi kullanıldı (p<0.05). Bulgular: Florozisli minede tam-seramik restorasyonların makaslama bağlanma direnci değerleri, florozis seviyesindeki artışa bağlı olarak azalma gösterdi. "TFI 0 ve TFI 4-6" ve "TFI 1-3 ve TFI 4-6" grupları arasında istatistiksel farklılıklar bulundu (p<0,05), "TFI 0 ve TFI 1-3" grupları arasında istatistiksel farklılık bulunmadı (p>0,05). Diş/restorasyon ara yüzeyleri arasında bağlanma başarısızlığı daha çok adeziv tipte gözlendi. Seramik diskler içinde koheziv tip kırığa rastlanmadı. Sonuç: Makaslama bağlanma direnci florozisin şiddetine bağlı olarak azalma gösterdi. Bağlanma başarısızlığı daha çok adeziv tip olarak izlendi. Florozisli minede tam-seramik restorasyonların, TFI 1¬ 3 seviyesindeki dişlerde kullanılabileceği, ancak TFI 4-6 seviyesindeki dişlerde bağlanma problemleri gözlenebileceği sonucuna varıldı.
