Chronic Calcaneal Actinomyces Osteomyelitis İn Children With Cauda Equina Syndrome: Case Report

Abstract (2. Language):
In a 10 years old male patient with cauda equina syndrome who has appealed to our clinic with pain, swelling and suppuration complains in his left heel, chronic osteomyelitis was diagnosed after the evaluation of clinical signs, laboratory masurements and radiographic evidence. In histopathological examination of the excised material after surgical operation, actinomyces osteomyelitis was diagnosed. No recurrence was observed during one year of follow up with suitable antibiotic tretament. Also, there was no local or systemic pathological sign. This case was found valuable to be represented, as osteomyelitis of the calcaneus is very rare in the literature and there is no other chronic osteomyelitis with actinomyces infection in a children with cauda equina syndrome so far.

Kliniğimize sol topuğunda ağrı, şişlik ve akıntı şikayetiyle başvuran 10 yaşındaki cauda equina sendromlu erkek hastada; klinik bulguları, laboratuar ve radyolojik tetkikleri sonucu kronik osteomiyelit saptandı. İncelemesinde başka bir enfeksiyon odağına rastlanmadı. Cerrahi tedavi uygulanan hastanın; ameliyat materyalinden alınan örneğinde patolojik inceleme sonucu aktinomiçes osteomiyelit tanısı konuldu. Uygun antibiyotik tedavisi verilen hastanın postoperatif bir yıllık takibinde nüks görülmedi ve lokal yada sistemik herhangi patolojik bulguya rastlanmadı. Literatürde kalkaneus osteomiyelitine az rastlanması ve cauda equina sendromu olan bir çocukta actinomiçes enfeksiyonunun belirlendiği kronik osteomiyelit olgusu bulunmaması nedeniyle sunulmaya değer görüldü.
