44 year-old female patient with a history of cough pain and exercise induced dyspnea admitted to our clinic. The chest x-ray revealed hyperlucency of the left lung. Computerized tomography with increased aeration and decreased vascular structures were observed on the left lung. There was a diffuse matched ventilation/perfusion defect on the left lung. MR angiography showed hypoplastic left main pulmonary artery and its branches. The patient was diagnosed as 'Unilateral hyperlucent lung syndrome-Swyer-James (Macleod's) Syndrome'. We wanted to present this rare case in the light of the current literature.

44 yaşında kadın hasta öksürük ve efor dispnesi yakınmasıyla kliniğimize başvurdu. Akciğer grafisinde sol hemitoraksta hiperlüsensi, akciğerlerin bilgisayarlı tomografisinde (BT) sol akciğerde havalanma artışı, damarlanmada ise azalma saptandı. Sintigrafide sol akciğerde diffüz, uyumlu ventilasyon/perfüzyon defekti mevcuttu. Pulmoner anjiografide sol ana pulmoner arter ve dallarında hipoplazi görüldü. Bu bulgularla hastaya Swyer-James (Macleod's) sendromu tanısı kondu. Bu nadir klinik olguyu literatür eşliğinde sunmak istedik.
