OBJECTIV E: The effect of uric acid and obesity on diurnal blood pressure variation in hypertensive patients is unknown. We aimed to investigate the specific relationship between uric acid, obesity and nocturnal dipping status essential hypertensive patients with normal renal function. Materi al and metho ds: Two hundred sixty-nine patients with newly diagnosed essential hypertension or on antihypertensive treatment underwent 24-hour ambulatory blood pressure monitoring, biochemistry analysis and urine testing. Res ults : Patients were classified as either dippers (108 patients, 40%) or nondippers (161 patients, 60%). Uric acid levels were higher in nondippers than in dippers (6.5 +/- 1.2 mg/dL vs. 5.5 +/- 1.1 mg/dL, p==0.001). In the same line, body mass index is significantly higher in nondippers compared to dippers (29.9±3.9 kg/m2 vs. 27.8±3.9kg/m2, p=0.01). ConclusIo n: This study demonstrates that increased serum uric acid levels and body mass index are associated with nondipping blood pressure patterns in patients with essential hypertension.

Amaç: Serum ürik asit düzeyinin ve şişmanlığın kan basıncının günlük ritmi üzerine etkisi net olarak halen bilinmemektedir. Bu çalışmada amaç, hipertansiyonu olan ve hipertansiyonu olmayan hastalarda serum ürik asit düzeyinin ve şişmanlığın kan basıncının günlük ritmi üzerine olan etkisini araştırmaktır. GEREÇ ve YÖN TEML ER: 269 adet yeni hipertansiyon tanısı konmuş veya antihipertansif tedavi alan hasta çalışmaya alındı. Bütün hastalara 24 saat ayaktan kan basıncı izlemi yapıldı ve kan değerleri kayıt edildi. Bulgular: Hastaların 108’i gece-düşen (%40) ve 161’i (60%) gece-düşmeyen olarak saptandı. Serum ürik asit düzeyleri gece-düşmeyen olanlarda gece-düşen olanlara göre anlamlı olarak yüksek saptandı (6,5 +/- 1,2 mg/dL ve 5,5 +/- 1,1 mg/dL, p==0.001). Aynı bağlamda beden kitle indeksi gecedüşmeyen olanlarda gece-düşen olanlara oranla yüksek saptandı (29,9±3,9 kg/m2 ve 27,8±3,9kg/m2, p=0,01). Sonuç: Bu çalışma serum ürik asit düzeyi ve beden kitle indeksinin hipertansiyonu olan hastalarda gece-düşmeyen kan basıncı modeli ile ilişkili olduğunu göstermiştir.
