Purulent pericarditis caused by pyogenic micro¬organisms is an acute, even catastrophic illness. Usually patients with this disease have symptoms and signs of a systemic toxic condition and hemody-namic compromise. The most common sources of in¬fection are extension from pulmonary disease, hema-togenous spread, penetrating chest wall injuries and extension from subdiaphragmatic suppurative lesi¬ons. We report two patients with pericardial tampo-nade and hypotension who were treated with hemo-dialysis due to chronic renal failure. Case one was a seventeen years old girl with car¬diac tamponade due to purulent pericarditis caused by Staphylococcus aureus. Case two was a 39 year old woman who underwent transplantation 9 years ago has rejected in the last 3 months. She was trea¬ted with hemodialysis regularly, developed cardiac tamponade due to septic pericarditis and died becau¬se of generalized septicemia.

Pürülan perikarditler pyojen mikroorganizmaların sebep olduğu kötü seyidi bir hastalıktır. Genellikle perikarditli hastalarda sistemik toksik bir tablo ve bo¬zulmuş hemodinamik bulgular mevcuttur. Sıklıkla akciğer hastalığınının sistemik yayılımı, hematojen yaydım, göğüs duvarının delici yaralan¬ması ve subdiafragmatik süpüratif lezyonlar sonucu gelişir. Biz burada kronik böbrek yetmezliği nedeniyle hemodiyaliz tedavisi görmekte iken hipotansiyon ve perikardiyal tamponad gelişen iki hastayı sunduk. tik vaka, Stafılokokkus aureus nedeniyle pürülan perikardit ve kardiak tamponad gelişmiş 17 yaşında bir kız hasta; ikinci vaka, 9yıl önce böbrek transplantasyonu yapılan ancak rejeksiyon nedeniyle 3 aydır düzenli hemodiyaliz tedavisi gören, bu sırada septik perikardite bağlı kardiak tamponad gelişen ve yaygın septisemi nedeniyle kaybedilen 39 yaşında bir kadın hasta.
