Prevention, treatment and rehabilitation of chronic diseases are important components of a family physician's practice. However, it is known that there are several problems regarding chronic care management. There is reasonably high percentage of undiagnosed chronic diseases in the community and a high percentage of diag¬nosed patients do not reach treatment targets. "Chronic care model" developed by Ed Wagner et al, aims to have better man¬agement of chronic diseases by using strategies, targets and protocols. This model includes 6 components: community linkages, heath care delivery system, self-management support, delivery system design, decision support and clinical information systems. This review deals with Chronic Care Model and presents evidence of efficacy when applied to primary care. Recent evidence supports that by implementing this model, improvements in parameters related with clinical practice and patient outcomes can be achieved.

Kronik hastalıklara yönelik korunma, tedavi ve rehabilitasyon süreçleri aile hekimliği pratiğinin önemli bir parçasıdır. Ancak kronik hastalıkların yönetiminde önemli sorunların olduğu bilinmektedir: Toplumda, kronik hastaların önemli bir bölümüne tanı konmamış, tanı konmuş olanların da önemli bir bölümünde tedavi hedeflerine ulaşılamamıştır. Ed Wagner ve arkadaşlarının geliştirdiği "kronik hastalık bakım modeli" strateji, hedef ve protokoller ile kronik hastalıkların daha iyi yönetilmesini amaçlamaktadır. Model 6 bileşenden oluşur; toplumsal kaynaklar ve politikalar, sağlık sisteminin organizasyonu, kendi kendine bakım desteği, hizmet sunum şekli, karar desteği ve klinik bilgi sistemleri. Bu derleme kronik hasta¬lık bakım modelini ele almakta ve birinci basamaktaki etkinliğine yönelik kanıtları sunmaktadır.
