The Effect of Different Sterilization Methods on Lymphocyte Subsets in Hemodialysis Patients

Biocompatibility of hemodialysis is mainly referred to the reactions induced by extracorporeal materials in contact with the blood. Data
regarding the effect of membran sterilization method on biocompatibility is limited. In this study, the effect of two polisulphon (PS)
membranes, which were sterilized with different methods, on the complement, CRP, peripheral total lympocyte count and subsets in
hemodialysis patients was investigated in a single dialysis session. 26 hemodialysis patients underwent hemodialysis with ethylene oxidesterilized PS (F5) membrane or with steam-sterilized PS (F5HPS) membrane for 4 hours. The aforementioned parameters were measured
from the blood samples taken before and after the dialysis session from all patients. In both groups, complement and CRP levels were
significantly increased following the dialysis. But there was no difference between the changes in two groups. While CD4 (helper/inducer
T cell) and CD4/CD8 rates were significantly increased in both groups, the changes in F5 and F5HPS groups were similar. CD8
(supressor/cytotoxic T cell) levels in F5 group and DR (activated T cell) levels in F5HPS group decreased significantly. Although the
changes in CD8 between two groups were not significant, the decrease in F5HPS group was significant compared to the increase in F5
group (-11.2% versus 23.7%). Our study suggested that the membran sterilization method in biocompatible syntethic membranes would
lead to mild changes in lymphocyte subsets.

Hemodiyaliz biyouyumluluğu başlıca kan ile ekstrakorporal materyalin temasının başlattığı reaksiyonları ifade etmektedir. Membran
sterilizasyon yönteminin biyouyumluluk üzerine etkisine ilişkin veri sınırlıdır. Bu çalışmada farklı yöntemlerle sterilize edilmiş iki
polisülfon (PS) membranın, hemodiyaliz hastalarında tek bir diyaliz seansında kompleman, CRP, periferik lenfosit sayısı ve lenfosit alt
grupları üzerine etkileri araştırıldı. 26 hemodiyaliz hastası etilen oksid ile sterilize edilmiş PS (F5) veya buhar ile sterilize edilmiş PS
(F5HPS) membran ile 4 saat hemodiyalize alındılar. Bütün hastalarda diyaliz seansından hemen önce ve bitiminde alınan kan örneklerinde
yukarıdaki parametreler ölçüldü. Her iki grupta diyaliz sonrası kompleman ve CRP düzeyleri anlamlı arttı. Fakat iki gruptaki değişiklikler
arasında fark yoktu. Her iki grupta da CD4 (helper/indüktör T hücre) ve CD4/CD8 oranı anlamlı artarken F5 ve F5HPS grubundaki deği-
şiklikler benzerdi. F5 grubundaki CD8 (supresör/sitotoksik T hücre), F5HPS grubunda DR (aktive T hücre) düzeyleri anlamlı azaldı. Her
iki grup arasında CD8’deki değişiklikler anlamlı olmamasına karşın F5HPS grubundaki azalma F5 grubundaki artış ile karşılaştırıldığında
anlamlı bulundu (%23.7’ye karşılık -%11.2). Çalışmamız membran sterilizasyon yönteminin biyouyumlu sentetik membranlarda lenfosit
alt gruplarında hafif değişikliklere yol açtığını düşündürmektedir.
