FREQUENCY OF RENAL OSTEODISTROPHY İN CHRONIC HEMODIALYSIS PATIENTS

In this study, a total of 111 chronic hemodialysis patients (53 male and 58 female) whose ages ranged from 11-75 years were studied in terms of renal osteo-distrophy (ROD). Parameters investigated included dialysis age, symptomatology (itching , muscle weak¬ness, extensive bone ache, spontaneous fracture), biochemistry (serum Ca, P, alk. phosphatase, PTH, serum aluminum levels) and radiology (Direct radiography and quantitative computerised tomography as¬sisted bone mineral density measurements). While serum Ca, P, alk. phosphatase and PTH values were not found to be significantly different in all dialysis age groups, the serum aluminum level was fo¬und to be singificantly related to dialysis age. The most commonly observed finding on direct ra¬diography was osteoporosis and this finding was fo¬und in 45% of cases. However, osetoporosis on bone mineral density measurements through computerized tomography was found to be present in 26% of cases. In the follow-up of these patients, bone mineral den¬sity measurements are of greater value.

Yaşları 11-75 arasında değişen 53 erkek 58 kadın toplam 111 kronik hemodiyaliz olgusunda renal oste-odistrofi (ROD) diyaliz yaşlarına göre gruplandırıla-rak semptomatolojik (kaşıntı, kas güçsüzlüğü, yaygın kemik ağrısı, spontan kırık), biyokimyasal (Serum Ca, P, alk. fosfataz, PTH, serum alüminyum düzeyleri) ve radyolojik (Direkt röntgen grafisi ve kantitatif bilgisa¬yarlı tomografi ile kemik mineral dansite ölçümleri) olarak araştırıldı. Olguların serum Ca, P, Alk. fosfataz, PTH değerle¬ri tüm diyaliz yaş gruplarında anlamlı farklı bulun¬mazken serum alüminyum düzeyi ve diyaliz yaşları arasında anlamlı ilişki saptandı. Yine alüminyum in-toksikasyonu riski ileri diyaliz yaş gruplarında daha yüksek oranda bulundu. Olguların direkt röntgen grafilerinde en sık rastla¬nılan bulgu olarak osteoporoz (ortalama %45) sap¬tandı. Buna karşın kantitatif bilgisayarlı tomografi ile kemik mineral dansite ölçümlerinde osteoporoz ol¬guların ortalama % 26' sında saptandı. Kemik mine¬ral dansite ölçümünün bu olguların takibinde daha değerli olduğu düşünüldü.
