THE EFFECTS OF PASSIVE SMOKING ON THE RESPIRATORY SYSTEM OF CHILDREN WHO APPLIED TO A FAMILY PRACTICE OUTPATIENT CLINIC

Objective: The aim of the present study was to evaluate the effects of exposure to passive household smoking on respiratory illness and to investigate the relationship between passive smok¬ing among children and the characteristics of patients, socio-eco¬nomic status, crowded homes, using a stove versus central heat¬ing.
Materials and Methods: The study was performed in Family Medicine Outpatient Clinics of Diyarbakır University Medical Faculty in 2003. Knowledge about indoor smoking habits, char¬acteristics of patients and demographic data were obtained from parents. A total of 124 children (72 males, 52 females), 55 with upper respiratory infection, 69 with lower respiratory infection, aged 0-14 years with a mean age of 39.5±45.4 months were evaluated. Logistic Regression Analysis was used to adjust for age, sex, household smoking, maternal or paternal smoking, recur¬rence of infection, socio-economic status, crowded house, using a stove for heating or central heating.
Results: Approximately 72% of children were presently exposed to passive household cigarette smoke at home. Both parents were all-time non-smokers in only 28.2% of the children. 17.8% of the mothers smoked over 20 cigarettes per day, however, this rate was 48.4 % for the fathers. 51.5% of the children lived in a crowded house (>6 subject). Risk factors that affected the cough were: both mother and father smoking, OR: 1.1 (%95 CI: 1.1¬1.3), smoking at home over 10 years, OR: 1.3 (%95 CI: 1.1-1.5), only one parent smoking, OR: 1.2 (%95 CI: 1.0-1.3), living in a home with over six inhabitants, OR: 1.2 (%95 CI: 1.0-1.3).
Conclusions: The use of tobacco products by adults with espe¬cially both parents smoking and overcrowded homes have enor¬mous adverse impacts on the respiratory system of children.

Amaç: Çalışmanın amacı pasif ev içi sigara içiciliğinin solunum yolu hastalıkları üzerindeki etkilerinin ve çocuklardaki pasif içicilik ile hastaların özellikleri, sosyo-ekonomik durumu, kalabalık evde yaşama, soba veya kalorifer kullanma arasındaki ilişkinin değerlendirilmesidir.
Gereç ve Yöntem: Çalışma 2003'te, Diyarbakır Üniversitesi Tıp Fakültesi Aile Hekimliği Polikliniği'nde yapıldı. Ev içi sigara içme alış¬kanlıkları, hastaların özellikleri ve demografik verileri ailelerden el¬de edildi. 55'i üst solunum yolu enfeksiyonu, 69'u alt solunum yolu enfeksiyonu tanısı alan yaş ortalaması 39.5±45.4 ay olan 0-14 yaş arası 124 çocuk (72 erkek, 52 kız) değerlendirildi. Yaş, cinsi¬yet, ev içi sigara içimi, anne veya babanın sigara içmesi, enfeksiyo¬nun tekrarı, sosyo-ekonomik durum, kalabalık ev, ısınmak için so¬ba veya kalorifer kullanımının etkisini belirlemek için lojistik regres-yon analizi kullanıldı.
Bulgular: Çocukların yaklaşık %72'si, son zamanlarda pasif ev içi sigara içiciliğine maruz kalmıştı. Çocukların yalnız %28.2'sinin anne ve babaları hiç sigara içmemişti. Annelerin %17.8'i günde 20'den fazla sigara içiyordu. Bu oran babalarda %48.4'tü. Çocukların %51.5' i kalabalık evlerde yaşıyordu (>6 kişi). En sık görülen belirti öksürüktü. Öksürük görülme riskini artıran faktörler: Anne ve babanın her ikisinin de sigara içmesi OR: 1.1 (%95 CI: 1.1-1.3), evde 10 yılı aşkın süredir sigara içilmesi OR: 1.3 (%95 CI: 1.1-1.5), babanın sigara içmesi OR: 1.2 (%95 CI: 1.0-1.3), evde >6 kişiden daha fazla kişinin yaşaması, OR: 1.2 (%95 CI: 1.0-1.3).
Sonuç: Tütün ürünlerinin yetişkinler tarafından kullanımı, özellikle de ev içinde anne ve babanın her ikisinin sigara içmesi ve kalaba¬lık, çocukların solunum sistemi üzerinde önemli olumsuz etkilere sahiptir.
