Bilateral thyroid operation concerns all of four parathyroid glands, which are in close contact with the capsule of thyroid. Hemithyroidectomy, which is a more limited operation can effect directly only two parathyroids settled on one side of the thyroid. That is why hypocalcemia rate after unilateral operation is lower. The aim of this study is to assess serum calcium changes and hypocalcemia after hemithyroidectomy. For this purpose, serum total calcium levels of 53 consecutive patients undergoing hemithyroidectomy procedure were obtained before the operation, between 12-24 hours after the operation, and at 1 month after surgery. When the data from these patients were examined, transient hypocalcemia developed in 12 (22.6%) patients, but all of these patients were asyptomatic. In conclusion, after hemithyroidectomy, serum total calcium level statistically decreases and transient hypocalcemia develops in a certain part of the cases, but it is usually asymptomatic. This situation can allow early patient discharge without a need for biochemical tests or calcium supplementation following the surgery

İki taraflı tiroid operasyonları tiroid kapsülü ile yakın temas halindeki dört paratiroid glandınıda ilgilendirir. Hemitiroidektomi daha sınırlı bir operasyon olup, sadece tiroidin bir tarafında yerleşik iki paratiroid glandını doğrudan etkileyebilir. Bu nedenle unilateral operasyon sonrası hipokalsemi oranı daha düşüktür. Bu çalışmanın amacı hemitiroidektomi sonrası gelişen serum kalsiyum değişiklikleri ve hipokalsemileri incelemektir. Bu nedenle hemitiroidektomi yapılan ardışık 53 hemitiroidektomi olgusunda operasyona giderken, operasyondan sonraki 12-24. saatlerde ve postoperatif 1.aydaki serum total kalsiyum düzeyleri incelenmiştir. Veriler incelendiğinde hemitiroidektomi sonrası 12 (%22.6) olguda geçici hipokalsemi gelişti, ancak bu olguların tümü asemptomatikti. Sonuç olarak, hemitiroidektomi sonrası total kan kalsiyum düzeylerinde anlamlı azalma olur ve olguların bir kısmında geçici hipokalsemi gelişir, ancak hipokalsemi genellikle asemptomatiktir. Bu durum cerrahi sonrası biokimyasal testlere veya kalsiyum desteğine gerek kalmaksızın hastaların erken taburcu edilmesini sağlayabilir
