Many different aproaches were reported for the reconstruction of columella defects that local fiaps through microvascular tissue transfer. There was a columella defects in 22 years old maie patient because of the explo-sion of bomb and the reconstruction was not thought by a local flap owing to large scar tissues next to the defect. After the dissection of wide adhesion of the septum nasi spongostan tampons were put into new plane. Then 2x1,5 cm diameter skin island flap with angular artery pedicul was prepared on the right nasolabial skin and transferred to the defect in order to the reconstruction of columella. The flap was folded and sutured. İt was made the flap thin by defatting 3 weeks laten The result was acceptable aesthetically. The results and short operative time for this flap were thought us that it is an alternafive to the microvascular reconstruction and morbidity of the donor area was acceptable.

Kolumella defekti onarımları için lokal fleplerden mikrovasküler doku aktarımlarına kadar bir çok farklı yaklaşım bildirilmiştir. 22 yaşında ve bomba patlaması sonucu oluşan kolumella defektli bir erkek hastada defekte komşu geniş skar dokuları sebebiyle lokal Hep ile onarım düşünülmedi. Yine aynı patlamaya bağlı septumda varolan yaygın yapışıklıklar açıldıktan sonra spongostan tamponlar açılan plana yerleştirildi. Ardından sağ nazoiabiyal bölgeden angüler arter pediküllü yaklaşık 2x1,5 cm'lik ada flebi hazırlanarak kolumella oluşturulmak üzere taşındı. Flep çift katlanarak sütüre edildi. 3 hafta sonra flebe "defatting" işlemi yapılarak inceltildi. Estetik açıdan oldukça kabul edilebilir bir sonuç elde edildi. Angüler arter pediküllü ada fiebinin sonuçları ve cerrahi süresinin kısalığı ile mikrocerrahiye iyi bir alternatif olacağı gibi donör alan morbiditesinin de oldukça kabul edilebilir düzeyde olduğu kanısındayız.
