Background/Aim: Obesity and cigarette smoking are important public health problems in the world. Both of them have effects in increasing morbidity and mortality. It has been shown that cigarette smoking decreases insulin sensitivity and increases fat accumulation in the abdominal region. The aim of our study was to compare the obesity parameters like body mass index, waist circumference, blood pressure, glycemic and lipid parameters in smoking and non-smoking morbidly obese women and evaluate the effect of smoking on these parameters retrospectively. Material and Methods: Eighty three female patients with a body mass index of  40 kg/m2 and without an accompanying illness that might lead to endogenous obesity were included in the study. Data was evaluated by dividing patients into two groups as smokers and non-smokers. Results: There was no statisticaly significant difference among the groups concerning mean age, body mass index, waist circumference, systolic and diastolic blood pressures, fasting blood glucose, insulin, total cholesterol, high density and low density lipoprotein cholesterol and triglyceride levels. Homeostasis Model Assessment of Insulin Resistance was found to be statistically significantly higher in smokers. Conclusion: In our study, we demonstrated that cigarette smoking increased insulin resistance. To more clearly put forward the effect of smoking on obesity, insulin resistance and related diseases, controlled prospective studies in similar patient populations are required.

Giriş ve Amaç: Obezite ve sigara kullanımı, dünyada en önemli halk sağlığı problemlerindendir. Her ikisinin de morbidite ve mortaliteyi artırıcı etkileri bulunmaktadır. Sigara kullanımının insülin duyarlılığını azalttığı, abdominal bölgede yağ depolanmasını artırdığı bilinmektedir. Çalışmadaki amacımız sigara içen ve içmeyen morbid obez kadınlarda vücut kitle indeksi ve bel çevresi gibi obezite parametrelerini, kan basıncı düzeylerini, glisemi ve lipid parametrelerini karşılaştırmak ve sigaranın bu parametreler üzerindeki etkisini retrospektif olarak incelemekti. Gereç ve Yöntem: Çalışmaya vücut kitle indeksi 40 kg/m2’nin üzerinde olan, endojen obezite yapabilecek yandaş hastalığı bulunmayan 83 kadın hasta dahil edildi. Verileri sigara içen ve içmeyen olarak iki gruba ayrılarak incelendi. Bulgular: İki grup arasında ortalama yaş, vücut kitle indeksi, bel çevresi, sistolik ve diyastolik kan basıncı, açlık kan şekeri, insülin, total kolesterol, yüksek ve düşük dansiteli lipoprotein kolesterol ve trigliserid düzeyleri bakımından anlamlı farklılık saptanmadı. Homeostasis Model Assessment of Insulin Resistance ile değerlendirilen insülin direnci sigara içenlerde anlamlı yüksek bulundu. Sonuç: Çalışmamızda sigaranın insülin direncini arttırdığını saptadık. Sigaranın obezite, insülin direnci ve ilişkili hastalıklar üzerindeki etkilerinin daha açık olarak ortaya konulabilmesi için benzer hasta popülasyonlarında kontrollü prospektif çalışmalara ihtiyaç bulunmaktadır.
