Posterior reversible encephalopathy syndrome is a radiological and clinical syndrome which is rarely seen in children. Hypertensive encephalopathy, renal failure, treatment with immunsuppressive agents, retansion of fluid, eclampsia, and transfusion are among the main etiological causes. In this report, we present the initial diagnostic and follow-up MR imaging findings of a 7 years old girl, who had been followed with chronic renal failure for 4 years, admitted to our pediatric department with symptoms of seizure and hypertensive episode, and diagnosed as having posterior reversible encephalopathy. On follow-up MR imaging, hemorrhage, a possible complication of this syndrome, was developed. Posterior reversible encephalopathy should be considered in the differential diagnosis of hypertensive children with seizures, and cranial MR and diffusion weighted MR imaging should be routinely performed in these patients.

Posterior reversibl ensefalopati sendromu çocuklarda nadir olarak görülen klinik ve radyolojik bir sendrom olup hipertansif ensefalopati, böbrek yetmezliği, immunsupresif ilaçlarla tedavi, sıvı retansiyonu, eklampsi, ve transfüzyon başlıca etiyolojik nedenler arasındadır. Bu raporda, dört yıldır kronik böbrek yetmezliği nedeniyle takip edilen, nöbet geçirme ve hipertansif atak ile çocuk polikliniğine başvuran ve yapılan MRG incelemesinde posterior reversible ensefalopati tanısı konulan yedi yaşındaki bir kız hastanın tanı ve takip MRG inceleme bulguları sunulmaktadır. Hastanın takip MRG incelemesinde posterior reversibl ensefalopati sendromunun olası komplikasyonlarından olan kanama gelişmiştir. Nöbet ile gelen hipertansif çocuklarda ayırıcı tanıda posterior reversibl lökoensefalopati sendromu düşünülerek beyin MRG ve difüzyon MRG incelemesi rutin olarak yapılmalıdır.
