Sudden Sensorineural Hearing Loss

Abstract (2. Language): 
The definition of sudden sensorineural hearing loss, an otolaryngology emergency, is a loss greater than 30 dB in three contiguous frequencies occurring in less than 3 days. Its major symptom is hearing loss, sometimes associated with tinnitus and vertigo. The spesific cause can be determined in only 15% of patients, thus it can be termed "idiopathic sensorineural hearing loss". Spontaneous recovery can be anticipated in 65% of the patients, but corticosteroids are the most commen and efficient treatment modality. Patients with mild hearing loss without vertigo and tinnitus, having early treatment and with evidence of odiologic remission in 2 weeks have better prognosis.

KBB acilleri içinde yer alan ani işitme kaybı, üç günden kısa bir zaman içinde gelişen, ardı ardına üç frekansta 30 dB'den daha fazla bir kayıpla ortaya çıkan sensörinöral işitme kaybı olarak tanımlanır. Major klinik bulgu işitme kaybıdır, ancak tinnitus ve başdönmesi de eşlik edebilir. Ani işitme kayıplarında spesifik etioloji vakaların ancak %10-15'inde saptanabilir. Bu nedenle "idiopatik ani işitme kaybı" olarak isimlendirmek de olasıdır. Ani işitme kayıplarında spontan iyileşme oranı %65'e kadar çıkmaktadır. Kortikosteroid tedavisi en yaygın kullanılan ve en etkili tedavi yöntemi gibi durmaktadır. Prognoz, hasta erken görülür ve tedaviye başlanırsa, iki hafta içinde odyolojik düzelmenin başladığı saptanırsa, semptomlar arasında tinnitus ve vertigo eşlik etmiyorsa ve odyogramda ağır olmayan bir işitme kaybı mevcutsa daha iyidir.
