Abstract (2. Language): 
A case of idiopatine myelofibrosis with ascites dııe lo peritoneal extramedullary hematopoiesis and portal hypertension. Extramedullary hematopoiesis is a constant feature of idiopathic myelofibrosis (IM). Seroas effusions ineluding ascites and plcural effusions can oecur but inf-requent. We presented a patient who was diagnosed as Evi and developed refractory ascites in the course of the diseasc. Portal hypertension and peritoneal extramedullary hematopoiesis were deteeted. Physicians should be aware of peritoneal involvemeııt in IM as demonstraled by ascitic fluid

İdyopatik myelofibroza (İM) eşlik eden ekstra medüller hematopoez birçok organda görülebilir, ancak scröz zarlarda görülmesi nadirdir. Burada idyopatik myelofibroz tanısıyla izlenirken refrakter asil gelişen bir hasta sunulmuştur. Hastada hem portal hipertansiyon hem de peritonda extramedüller hematopoezin varlığı gösterilmiştir. Extramedüller hcmatopoezc bağlı asit kemoterapi ya da radyoterapi ile tedaviye yanıt verebileceğinden ÎM seyri sırasında gelişen asitİn ayırıcı tanısı için sitolojik yaymasının mutlaka yapılması gereklidir
