AKROMEGALİDE ADRENALADENOM VE İDRAR KATEKOLAMİNLERİNDE YALANCI ARTIŞ: OLGU SUNUMU

Acromegaly is a known risk factor for ischemic heart disease. An adrenal adenoma along with increased urine catecholamine levels is highly indicative of pheochromocytoma. However, ischemic heart disease can cause increment in the urine catecholamines. Herein, we present a case having classic features of acromegaly and ischemic heart disease. On his thorax computed tomography scan performed for paracardiac opacity on X-ray, there was an adrenal adenoma and his urine catecholamine levels were high. After he underwent coronary by-pass grafting operation, his urine catecholamine levels returned to normal. If clinical probability of pheochromocytoma is low in the setting of increased urine catecholamines, other sympathic activation causes like ischemic heart disease, should be considered and excluded before pheochromocytoma is diagnosed.

Akromegali, iskemik kalp hastal›¤› için bilinen bir risk faktörüdür. Adrenal adenom ile birlikte idrar katekolaminlerinde art›fl olmas› durumunda feokromositoma ihtimali oldukça yüksektir. Bununla birlikte, iskemik kalp hastal›- ¤›na ba¤l› olarak da idrar katekolaminlerinde art›fl ortaya ç›kabilir. Yaz›m›zda, akromegalinin klasik özelliklerini bar› nd›ran ve iskemik kalp hastal›¤› olan bir olgu sunmaktay›z. Olgumuzda posteroanterior akci¤er grafisinde saptanan parakardiyak opasitenin incelenmesi için çekilen toraks bilgisayarl› tomografisinde (BT) bir adrenal adenom tespit edilmifl ve idrar katekolamin seviyeleri yüksek bulunmufltur. Koroner arter by-pass greft operasyonu sonras› nda idrar katekolamin seviyeleri normale dönmüfltür. ‹drar katekolaminleri yüksek bulundu¤unda, klinik olarak feokromositoma ihtimali düflük ise, iskemik kalp hastal›¤› gibi sempatik aktivasyonun bulundu¤u durumlar akla getirilmeli ve feokromositoma tan›s› konulmadan önce bu durumlar d›fllanmal›d›r.
