Abstract (2. Language):
The iron in the hemoglobin molecule is in ferrous state (Fe++). The oxidation of iron from ferrous to ferric (Fe+++)
state causes methemoglobinemia. This reaction decreases the oxygen-carrying capacity of hemoglobin leading to
hypoxemia of the peripheral tissues and even to death in severe cases. Methemoglobinemia is frequently related to
chemical substances. However, hereditary or idiopathic cases have also been described. Methemoglobinemia due
to local anesthetic drugs may occur especially in newborn infants. Here, we presented an adult case in which methemoglobinemia
developed after the administration of prilocaine prior to an epilation procedure

Normal flartlarda hemoglobinde bulunan demir iki de¤erlidir (Fe++). Hemoglobindeki demirin okside olup üç de¤erli
(Fe+++) duruma geçmesiyle methemoglobinemi oluflur. Bu reaksiyon, dokuda hipoksemiye yol açar. A¤›r vakalarda
hemoglobinin, oksijen ve karbondioksit tafl›ma kapasitesindeki ciddi bozulma nedeniyle ölüm meydana gelebilir.
Methemoglobinemi s›kl›kla kimyasal maddelerle temas sonras› görülmekle beraber, genetik, besinlere ba¤l› ve
idiopatik olarak da bulunabilir. Lokal anestetiklere ba¤l› methemoglobinemi daha çok yenido¤anlarda karfl›lafl›lan
bir durumdur. Bu yaz›da epilasyon ifllemi öncesi prilokain uygulanan ve sonras›nda akut methemoglobinemi geliflen
eriflkin bir olgu sunulmufltur.
