Our First Experiences in 23 Gauge Transconjunctival Sutureless Vitrectomy

Objective: To determine anatomic and functional results and the complications of 23Gauge (G) transconjunctival sutureless vitrectomy technique
before and after surgery.
Materials and Methods: Records of 40 eyes of 40 patients in which underwent 23 G PPV were screened. Intra- and postoperative complications as
well as pre- and postoperative visual acuity, intraocular pressure (IOP), slit-lamp and fundus examination and anatomical results of the surgery were
reviewed.
Results: The mean age of the patients was 59.24±1, 2 years (Range:19-80 years). The mean follow-up of the patients were 9, 3 months. Surgery
indications for 23G PPV were VH, secondary vitreous condansation, ERM and endophthalmitis. The median visual acuity was 1,7 logmar
preoperatively, and was 1,0 logmar visual acuity in follow-up of 6 months postoperatively. Suturation was not needed in any eyes. Subconjunctival
hemorrhage (25%) was the most common intraoperative complication. There was not increase in IOP postoperatively. No choroidal detachment was
noted in any eyes. None of the eyes developed endophthalmitis. In only two eye, retinal detachment was developed and it was needed to convertion to
20 G PPV. Hyphema (10%) and vitreous re-hemorrhage (15%) were the most common postoperative complications.
Conclusion: It was found that 23 G PPV technique is effective and safely technique achieving successful anatomic and functional outcomes for
selected cases. We consider that we will obtain more successful results when our surgical experiences are getting improve and that our indications for
the use of this technique will enlarge if we can obtain to use of the new instruments developed for 23 G PPV.

Amaç: 23 gauge (G) transkonjonktival dikişsiz pars plana vitrektominin (PPV) anatomik ve fonksiyonel sonuçlarını, ameliyat sırasındaki ve sonrasındaki
komplikasyonlarını değerlendirmektir.
Gereç ve Yöntemler: 23 G dikişsiz PPV cerrahisi uygulanan 40 hastanın 40 gözüne ait dosyalar geriye dönük olarak incelendi. Ameliyat öncesi ve
ameliyat sonrası takip muayenelerindeki görme keskiniği ve göz içi basıncı (GİB) değerleri, biyomikroskopi ve göz dibi muayene bulguları ile birlikte
ameliyat sırasındaki ve sonrasındaki komplikasyonlar ve cerrahinin anatomik ve fonksiyonel sonuçları incelendi.
Bulgular: Hastaların yaş ortalaması 59.24± 1,2 yıl idi (19-80). Ortalama takip süresi 9,3 aydı. Cerrahi endikasyonlar vitreus hemorajisi (VH),
epiretinal membran (ERM), sekonder vitreus kondansasyonu ve endoftalmitis idi. Ameliyat sırasında 10 gözde (%25) subkonjonktival kanama gelişirken
olguların hiçbirinde kesi yerinden sızıntı izlenmedi. Ameliyat sonrası dönemde hiçbir olguda GİB yükselmesi veya hipotoni, koroid dekolmanı
ve endoftalmitis gelişmedi. Dört gözde (%10) ameliyat sonrası birinci gün ön kamaraya kanama, iki gözde (%5) ön kamarada yoğun enflamatuar
hücre izlendi. İki gözde ise (%5) ameliyat sonrası erken dönemde katarakt gelişti. Vitreus hemorajisi nedeniyle ameliyat edilen altı gözde (%15) VH
tekrarladı. İki gözde retina dekolmanı gelişmesi (%5) üzerine kesi yerleri genişletilip 20 G PPV'e geçildi. Ameliyat öncesi ortalama görme keskinliği
1,7 logmar düzeyinde olup ameliyat sonrası 6. aydaki takipte 1 logmar düzeyine yükseldi.
Sonuç: 23 G dikişsiz PPV tekniği, seçili vakalarda etkin ve güvenilir bir yöntem olup anatomik ve fonksiyonel başarı sağlanmıştır. Cerrahi tecrübemiz
arttıkça komplikasyon oranlarımız azalacak ve bu teknik için geliştirilen cerrahi aletleri kullanma olanağı bulduğumuzda 23 G PPV için
endikasyon yelpazemiz genişleyecektir.
