In this study ouı aim was to cvaluate the effectivcness o bilateral medial reetus muscle sıırgery as an al tema ti vc treatment for convergence excess type of esotropia. Medical records of 29 patients who under-went surgery for the convergence excess type of esotropia. Medical records of 29 paîients who underwent surgery for the convergence excess type of esotropia with high AC/A ratio and dis-tance near disparity of at least lOAD's were studied retrospeetively. Ali patients received bimedial reetus muscle recession of 3 to 6 mİIIİmeters to correct the near csodeviation measured with full hyperopic correetion. After a follow-up period of at least 1 year, the magnitude of the aiignment, AC/A ratio, and the state of binocularity were measured. After a postoperative fol-low-up period of at least one year two patients needed a reoperation due to inadequate aiignment. Overall with 1.1 operations per palient the mean preoperative esodeviation at near redu-ced from 30.34 AD (SD±11.92) to 9.76 AD (SD+7.54) and none of the patients needed bifocal OT miotİcs after surgical aiignment. These results demonslrate that bimedial surgery is an efec-tİve alternative to the long-term use of bifocals and miotics in the treatment of convergence excess type of esotropia.

Konverjans fazlalığı tipindeki ezotropiaSarda bir tedavi alternatifi olan bilateral iç rektus geriletme cerrahisinin etkinliği araştırmak amacıyla AC/A oranı yüksek ve uzak - yakın arası fark lOAD'den fazla olan ve cerrahi uygulanan 29 hastanın dosyalan retrospektif olarak incelendi. Hastaların hepsinde tam hiperopik düzeltme ile ölçülen yakın ezotropiayı düzeltmek amacıyla iç rektus kaslarma iki taraflı 3-6 milimetre arasında geriletme uygulanmıştı. En az bir yıl olan takip süresi sonunda düzelme derecesi, AC/A oranı ve binoküler görmenin düzeyi belirlendi. Cerrahi sonrası bir yıllık takip sonunda yetersiz düzeltme sebebi İle 2 hastada ek bir cerrahi girişim ihtiyacı duyulmuştu. Hastalarda yakın bakışta preoperatif ortalama 30.34 AD (SD± 11.92) olan ezotropi miktarı postoperatif 9.76 AD'e (SD+7.54) düşmüş ve hastaların hiçbirisinde cerrahi düzeltme sonrası bifokal veya miyotik tedaviye ihtiyaç duyulmamıştı. Sonuçlarımız konverjan fazlalığı tipi ezodeviasyonun tedavisinde iki taraflı iç rektus geriletmesinin bifokal miyotiklerin uzun süreli kullanımına alternatif bîr yöntem olabileceğini göstermiştir.
