Adult onset still's dısease admitted wıth mononucleosıs syndrome like symptoms. Fifty years old male, admitted to our clinic with intermittant fever, macular rash, asymetric arthralgia and sore throat. His symptoms was started five months ago vvith macular rash beginning from fore-head and than spreading to whole body and sore throat. Soon arthralgia and intermittant fever spiking in the evening were occured. He was treated vvith several antibiotics as upper respira-tory tract infection but his symptoms were enhanced in time. We found high fever, servical lymphadenopathy and percutable spleen. In blood smear, there was an increased number of polymorphonuclear leukocytes and few atypic lymphocytes. His serum ferritin level was high. Anti nuclear antibody, anti ds DNA, other auto antibodies and viral serologic parameters were negative. With these data we diagnosed adult onset Still's di-sease. With indomethacin, methyl prednisolone and low-dose oral methotrexate treatment his symptoms were recovered. Gradual steroid dose reduction was planned and he was externali-sed.

50 yaşında erkek hasta, beş ay önce alında ortaya çıkıp tüm vücuda yayılan maküler tarzda döküntü ve boğaz ağrısı nedeni ile başvurduğu doktorlar tarafından üst solunum yolu infeksiyo-nu tanısı konulmuş ve çeşitli oral antibiyotik tedavileri kullanmış. Zamanla yüksek ateş ve eklem ağrıları ortaya çıkmış. Kliniğimize başvurduğunda servikal lenfadenopati, eklemlerde hassasiyet, tekrarlayıcı nitelikte yüksek ateş, ateş dönemlerinde ortaya çıkan maküler tarzda döküntüler saptandı ve traube alanı kapalı bulundu. Çevresel kanda belirgin polimorfonükleer lö-kosit artışı ve seyrek atipik lenfosit görüldü. Ferritin değeri çok yüksekti. Tüm serolojik incelemelerle otoantikor araştırmaları ve viral göstergeler negatif bulundu. Erişkin tip Stili Hastalığı tanısı konuldu. İndometasin, metil prednisolon ve düşük doz oral metotreksat tedavisi ile kısa sürede şikayetlerinde gerileme oldu. Steroid dozu düşürülerek oral forma geçildi ve Roma-toloji Bilim Dalı polikliniğinden takip edilmek üzere hastaneden çıkarıldı.
