Granulocytic sarcoma is a rare extramedullary tumor that originates from the primitive granulocytic cells. Granulocytic sarcoma develops during the courses of acute myleoid leukemia, myelodisplastic syndrome and myeloproliferative diseases and frequently it is observed on the bones, soft tissue, periost, lymph nodes and skin. Here we report a 60 year old female patient who is at first diagnosed as diffuse large B cell lymphoma according to the biopsy result of the 6x10 cm nodular lesion on the anterolateral side of her left calf and at last diagnosed as granulocytic sarcoma upon reevaluation of the biopsy material. Granulocytic sarcomas are frequently confused with lymphomas or the sarcomas. Here the difficulties of the diagnosis of granulocytic sarcoma is discussed and reviewed in association with the literature.

Granülositik sarkom; primitif granülositik hücrelerden köken alan, nadir görülen, ekstramedüller bir tümördür. Granülositik sarkom; akut myeloid lösemiler, myelodisplastik sendrom ve myeloproliferatif hastal›klar›n seyrinde geliflir ve s›kl›kla kemik, yumuflak doku, periost, lenf nodlar› ve deride görülür. Biz burada, sol bald›r ön-yan yüzde lokalize, 6x10 cm. boyutlar›nda, nodüler görünümlü, biyopsi incelemesinde önce diffüz büyük B hücreli lenfoma tan›s› alan, ancak biyopsi materyalinin yeniden de¤erlendirilmesi socunda granülositik sarkom tan›s› konan 60-yafl› nda bir kad›n hastay› bildiriyoruz. Granulositik sarkomlar tan› aflamas›nda s›k olarak lenfomalar veya sarkomlar ile kar›flabilmektedir. Burada, granülositik sarkom tan›s› konulurken, karfl›lafl›lan güçlükler irdelendi ve literatür eflli¤inde yeniden gözden geçirildi
