EVALUATION OF CARBOHYDRATE AND LIPID METABOLISM IN PEDIATRIC PREDIALYSIS AND CAPD PATIENTS

This study was designed to evaluate the effects of CAPD and/or uremia on carbohydrate (ch) and lipid metabolism in pediatric chronic renal failure (predialysis (PD) and CAPD) patients, compared to an age-matched controls. Comparisons of fisting/postprandial glucose (mg/dl) and insulin (mu/l) levels, insulin to glucose ratios and lipid and lipoprotein (mg/dl) levels were made among 3 groups. Blood glucose and insulin levels were within normal limits in all patients. Hyperinsulinemia was detected (FI/FG<0.3, PI/PG<0.5) neither in PD, nor in CAPD patients. Peritoneal dialysis patients had significantly elevated levels of TG, Cho and LDL , whereas levels of HDL were normal. We conclude that pediatric patients treated with CAPD have atherogenic lipid profiles, but no abnormalities of ch metabolism. Since sufficient data are not available for pediatric CAPD population and existing reports are still controversial, further evaluation and controlled studies are needed.

Bu çalışma kronik böbrek yetmezliği (KBY) olan çocuklarda; diyaliz öncesi (PD) dönemde ve sürekli ayaktan periton diyalizi (SAPD) tedavisi uygulanırken, karbohidrat (KH) ve lipid metabolizmalarını değerlendirmek amacıyla planlanmıştır. Sürekli ayaktan periton diyalizi, PD hastaları ve aynı yaş grubunda yer alan sağlıklı çocuklardan oluşan kontrol grubunda açlık, tokluk kan şekeri (mg/dl), insülin düzeyleri (mu/l), insülin/glukoz oranları ile plazma lipid ve lipoprotein (mg/dl) düzeyleri karşüaştınlmıştır. Tüm gruplarda kan şekeri ve insülin seviyeleri normal olup, hiperinsülinizm saptanmamıştır (Aİ/AG<0.3, Tİ/TG<0.5). Periton diyalizi hastalarındaki önemli ölçüde yüksek TG, Cho ve LDL düzeylerine karşın, HDL düzeyleri her üç grupta da normal sınırlarda bulunmuştur. Bu verilerle SAPD tedavisinin çocuklarda aterojenik lipid profili oluşturduğu, buna karşın KH metabolizması üzerine negatif etkisinin olmadığı düşünülmüştür. Pediatrik SAPD hastalarında bu konuda yeterli veri yoktur ve mevcut sonuçlar tartışmalıdır. Daha ayrıntılı araştırmalara ve kontrollü çalışmalara gereksinim vardır.
