The Results of Total Knee Replacement in Knees with Severe Varus Deformity

The purpose of the study is comparing the clinical and the radiographic results of total knee replacement (TKR) performed on severe varus knee deformity using a smaller and lateralized tibial component and TKR on patients with no evident angular deformity as a control group. We investigated a series of TKR patients who were operated between February 2003 and January 2008 in our clinic with the study group consisted of 23 knees of 15 patients with preoperative varus angular deformity greater than 20°, and the control group consisted of 182 knees of 156 patients with less than 10° varus deformity and less than 15º flexion contracture. A statistically significant difference was determined between pre-and postoperative knee scores in the severe varus group (p<0.001). A statistically significant difference was determined between pre-and postoperative knee evaluation scores in the control group also (p<0.001). According to these results, due to the advantages brought by this technique in achieving an acceptable mechanical axis space in knees with severe varus deformity, and meeting the clinical and functional expectations of the patients, total knee arthroplasty can be performed successfully

Çalışmanın amacı, ileri varus deformiteli dizlerde bir küçük boy tibial komponent kullanılması ve komponentin lateralizasyonu ile uygulanan total diz protezi (TDP) sonuçlarını klinik ve radyografik olarak değerlendirmek ve bu sonuçları ameliyat öncesi belirgin açısal deformitesi olmayan kontrol grubu ile karşılaştırmaktır. Kliniğimizde, Şubat 2003-Ocak 2008 yılları arasında çalışma grubu olarak, ameliyat öncesi varus açısal deformitesi 20° den büyük 15 hastanın 23 dizine ve kontrol grubu olarak, ameliyat öncesi 10° den küçük varus deformitesi olan ve 15° den az fleksiyon kontraktürüne sahip 156 hastanın 182 dizine TDP uygulaması gerçekleştirildi. İleri varus grubunda, ameliyat öncesi ve sonrası diz değerlendirme skorları arasında istatistiksel olarak anlamlı fark gözlendi (p<0,001). Kontrol grubunda ameliyat öncesi ve sonrası diz değerlendirme skorları arasında istatistiksel olarak anlamlı fark gözlendi (p<0,001). Bu sonuçlara göre ileri varus deformiteli dizlerde kabul edilen mekanik aks aralığının sağlanması, klinik ve fonksiyon açısından hastaların beklentilerinin karşılanması ve uygulamış olduğumuz tekniğin getirmiş olduğu avantajlar sayesinde yapılan total diz protezi ameliyatlarının başarılı bir şekilde uygulanabileceğini düşünmekteyiz.
