Simple renal cysts are asymptomatic incidental findings; however, for a small subset of benign renal cysts, patients may present with pain, hematuria, urinary infection, pyelocaliceal obstruction or hypertension. Laparoscopic cyst ablation is an effective minimally invasive modality for the treatment of benign renal cyst. Our aim was to show the ultrasonic energy safe and effective method during laparoscopic renal cyst excision. In our clinic 14 patients underwent operated laparoscopic renal cyst excision with transperitoneal approach between January 2006 and February 2010. Trocar port for camera was inserted to the peritoneum in order to open the told line, gerato fascia and fat over the cyst. Harmonic™ scissor with the cautery was used to incise the cyst wall. Mean age of the patients was 58,1 years. Mean operation and hospitalization times were 59,2 minutes and 1,57 days, respectively. We did not need the fourth trocar during operation and the operation was finished. There was no complication in this procedure. Mean follow-up time was years. In long-term follow-up period symptomatic recurrence was observed in one patient and someone else be observed radiologic recurrence. We believe that using ultrasonic device is safe and effective method in laparoscopic cyst decortication. However, prospective controlled trials are needed to evaluate the other energy sources.

Toplumda sık olarak gözlenen basit böbrek kistleri genellikle asemptomatikdirler ancak bazen yan ağrısı, hipetansiyon, idrar yolu enfeksiyonu ve toplayıcı sisteme bası gibi semptomlar verirler. Laparoskopi bu kistlerin tedavisinde iyi tanımlanmış bir yöntemdir. Bu çalışmada laparoskopik kist dekortikasyonu sırasında ultrasonik enerjinin güvenilirliği ve etkinliği gösterilmek istenmiştir. Kliniğimizde ocak 2006 ve şubat 2010 yılları arsında toplam 14 hastaya böbrek kisti nedeniyle transperitoneal laparoskopik kist dekortikasyonu uygulanmıştır. Periton içine girildikten sonra told hattı açılmış gerota fasyası açılarak kist ortaya konulmuştur, daha sonra kist duvarının kesme ve mühürleme işleminde ultrasonic enerji ile çalışan harmonic™ cihaz kullanılmıştır. Hastaların ortalama yaşı 58.1 idi ve hastaların 8’si kadın 6’sı erkek idi. Operasyon süresi 59.2 hastanede kalış süresi ort 1.57 olarak bulundu. Hiç bir hastada 4. trokara ihtiyaç kalmadan operasyon tamamlandı ve hiç bir hastada komplikasyon gelişmedi. Uzun dönem takiplerinde 1 hastada semptomatik nüks görülürken yine 1 hastada radyolojik nüks gözlendi. Ultrasonik enerji kullanarak laparoskopik kist dekortikasyonun etkin ve güvenilir bir yöntem olduğunu düşünmekteyiz ancak diğer enerji kaynaklarınıda değerlendirecek karşılaştırmalı çalışmalara ihtiyaç vardır.
