ABSTRACT Objective: To evaluate the diagnostic accuracy of transvaginal ultrasonography, sonohysterography and hysteroscopy in patients presenting with postmenopausal bleeding. Materials and methods: In a prospective study, 50 women with postmenopausal bleeding underwent conventional ultrasound examination and saline contrast sonohysterography. All diagnoses established by transvaginal ultrasonography, and sonohysterography were compared to the appearance of the cavity using hysteroscopy as well as the histological diagnosis from removed tissues. Results: The results for detection of an abnormal uterine cavity were as follows: sensitivities of transvaginal ultrasonography, sonohysterography and hysteroscopy were 100%; specificities of transvaginal ultrasonography was 76%, sonohysterography was 76% and hysteroscopy was 100 %; positive predictive values of transvaginal ultrasonography was 89%, sonohysterography was 89%, and hysteroscopy was 100%. Conclusion: Sonohysterography is as good as ultrasonography in detecting focally growing lesions in the uterine cavity in women with postmenopausal bleeding.

ÖZET Amaç: Postmenopozal kanamalarda transvaginal ultrasonography (TVS) sonohisterografi ve histereskopinin tan› koymada yararl› olup olmad›¤›n›n araflt›r›lmas›. Gereç ve yöntem: Prospektif olarak yap›lan bu çal›flmada 50 hastaya transvajinal ultrasonografi, sonohisterografi ve histeroskopi uygulanm›flt›r. Tüm tan›lar histeroskopi sonras›nda yap›lan biyopsi sonuçlar›yla karfl›laflt›r›lm›flt›r. Bulgular: Tüm endometrial lezyonlar gözönüne al›nd›¤›nda endometriumda patoloji saptamada sensitivitesi TVS, sonohisterografi ve histeroskopi için % 100, spesifisite TVS ve sonohisterografi için % 76, histeroskopi için % 100, TVS ve sonohisterografi için % 89, histeroskopi için % 100 bulunmufltur. Sonuç: Sonohisterografi en az TVS kadar postmenopozal kanamalarda endometrial patolojilerde de¤erlidir.
