Miyasteni Gravis Tedavisinde Genişletilmiş Timektomi

Extended transsternal thymectomy was carried out in 10 patients with myasthenia gravis. There were 2 males and 8 females. Thymic hyperplasia was found in 7patients, athrophic thymus in 1 and thymoma in 2. In the nonthymomatous myasthenia gravis patients complete remission was achieved in 4 patients (40%), pharmacological remission in 3 patients (30 %) and improvement in 1 patient (10%). One patient (10%) with thymoma died because of respiratory failure. Another patient (10%) with thymoma has improved. In conclusion, clinical improvement was obtained in 90% of our patients following extended transsternal thymectomy. [Journal of Turgut Özal Medical Center 1997;4(1):89-92]

Miyasteni gravis tanısıile 10 hastaya transsternal yolla genişletilmiş timektomi uygulandı. Hastaların 2'si erkek 8'i kadın idi. İki hastada timoma, 7'sinde timus hiperplazisi saptandı; 1 hastada timus bezi atrofikti. Timoma dışı miyasteni gravis saptanan 4 (%40) olguda tam remisyon, 3 (%30) hastada ilaçla remisyon, 1 (%10) hastada ise ameliyat öncesi döneme göre belirgin iyileşme sağlandı. Timomalı 1 (%10) olgu ameliyat sonrası solunum yetmezliği nedeniyle kaybedildi, diğer olguda (%10) cerrahi sonrası belirgin iyileşme sağlandı. Sonuç olarak, genişletilmiş transsternal t ime kt om iyi takiben olguların %90'ında klinik düzelme gözlendi. [Turgut Özal Tıp Merkezi Dergisi 1997;4(l):89-92]
